研究目的：

1.通过病人门户系统构建患者连续个人电子健康记录和慢病管理平台，实时上传和下载个人的病史和医疗信息，提供居家慢病管理服务、完成门诊-住院-随访的整体医疗程序的促进。 2.通过随机对照试验，评价该慢病管理平台对高血压的管理效果，了解平台对高血压慢病患者的自我管理能力、服药依从性、生活质量的影响。

Objectives of Study：

1. Build a continuous personal electronic health record and chronic disease management platform for patients through the patient portal system, which can upload and download personal medical history and medical information in real-time, providing home-based chronic disease management services and facilitating the overall medical procedure of outpatient, hospitalization, and follow-up. 2. Evaluate the operational effectiveness of the platform on chronic hypertension management through a randomized controlled trial, and understand the impact of the platform on the self-management ability, medication compliance, and quality of life of patients with chronic hypertension.

Inclusion criteria

排除标准：

1.伴有严重新、脑、肾脏等综合疾病或伴有严重并发症； 2.伴有脏器或血管进展性损伤； 3.缺乏自理能力或存在明显认知、语言和听力障碍； 4.其它精神疾病； 5.家属有明显反对。具备以上任何一条均排出。

Exclusion criteria：

1. Accompanied by comprehensive diseases such as severe illness, brain, kidney, or serious complications 2. Accompanied by progressive damage to organs or blood vessels; 3. Lack of self-care ability or significant cognitive, language and hearing impairment; 4. Other mental diseases; 5. The family members have obvious objections. Any one of the above is discharged.

一名不参与本研究的护士利用SAS系统PROCPLAN过程语句，给定种子数并列出流水号为001-320号。将001-320序号通过随机分配数字表法产生匹配的随机数字，规定随机数字为偶数者分到A组，奇数者分到B组。

A nurse who was not involved in this study used the SAS system PROCPLAN procedure statement to give the number of seeds and list the serial numbers as 001-320. Using the method of randomly assigning numbers from 001 to 320 to generate matching random numbers, it is specified that those with even numbers are assigned to group A and odd numbers are assigned to group B.
