研究目的：

本研究拟在长期护理保险政策下探索脑卒中患者延续护理模式方案的制订，以促进患者功能的最大恢复和生活质量的提高，同时推动长期护理保险制度的落实，亦为优化照护服务资源配置提供重要依据。  

Objectives of Study：

This study intends to explore the formulation of a continuation care model for stroke patients under the long-term care insurance policy, so as to promote the maximum recovery of patients' function and improve the quality of life, and promote the implementation of the long-term care insurance policy, and also provide an important basis for optimizing the allocation of nursing service resources.

纳入标准：

Inclusion criteria

排除标准：

1. 患有严重的急性疾病或处于慢性疾病急性发作期者； 2. 心、肝、肾等严重器官功能障碍及恶性肿瘤患者。

Exclusion criteria：

1. Suffering from severe acute diseases or in the acute attack stage of chronic diseases; 2. Heart, liver, kidney and other severe organ dysfunction and malignant tumor patients.
