研究目的：

采用前瞻性临床研究方法，探讨缺血预处理对肝大部切除术中缺血再灌损伤的保护作用，并比较肝脏直接缺血预处理同远端缺血预处理保护效应的大小。  

Objectives of Study：

To investigate the protective effect of ischemic preconditioning on ischemia reperfusion injury in subtotal hepatectomy, and to compare the protective effect between direct and distal hepatic ischemic preconditioning.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

Inclusion criteria

排除标准：

1. 有周围血管疾病影响上肢功能；
2. 严重肝硬化；
3. 手术过程与预计方式不同；
4. 既往有肝脏手术病史；
5. 术中未阻断肝门静脉或累计阻断时间超过60 min；
6. 出血量大于4000 ml；
7. 合并其他主要器官系统疾病如心脏梗死、慢性阻塞性肺疾病、脑梗死；
8. 术前心肺肾功能及电解质明显异常；
9. 服用格列本脲、磺脲类的糖尿病患者；
10. 术中使用糖皮质激素等抗炎药物；
11. 有内分泌疾病及阿片类药物成瘾史；
12. 既往有肝脏介入、射频或者放疗、化疗史；
13. 合并精神病、肺部感染等。

Exclusion criteria：

1. Peripheral vascular disease affecting upper limb function;
2. Severe liver cirrhosis;
3. The surgical procedure was different from the predicted method;
4. Previous history of liver surgery;
5. The hepatic portal vein was not blocked during the operation or the cumulative blocking time was more than 60 minutes;
6. Blood loss > 4000 ml;
7. Combined with other major organ system diseases, such as heart infarction, chronic obstructive pulmonary disease, and cerebral infarction;
8. Preoperative cardiopulmonary, renal function and electrolytes were significantly abnormal;
9. Diabetic patients taking glyburide and sulfonylureas;
10. Intraoperative use of glucocorticoids and other anti-inflammatory drugs;
11. History of endocrine diseases and opioid addiction;
12. Previous history of liver intervention, radiofrequency, radiotherapy and chemotherapy;
13. Combined with mental illness, lung infection, etc.
