研究目的：

1、评估基于个人和家庭自我管理理论（IFSMT）应用于2型糖尿病自我管理行为干预研究的可行性，探究行为综合干预对2型糖尿病患者的疾病控制与生活质量的影响。 2、基于E-health平台（微信小程序），在疾病常规管理服务的基础上，通过系统的健康教育课程、个性化信息提示等管理方式，开展2型糖尿病患者的自我管理行为和血糖控制的干预研究，探究移动电子技术应用于2型糖尿病患者的可行性，评估E-health干预对患者自我管理行为和血糖控制的影响。  

Objectives of Study：

1. To evaluate the feasibility of applying the Individual and Family Self-Management Theory (IFSMT) to the research on self-management behavioral intervention in type 2 diabetes, and to explore the effect of comprehensive behavioral intervention on disease control and quality of life in patients with type 2 diabetes. 2. Based on the E-health platform (Wechat applet) , on the basis of routine disease management services, systematic health education courses, personalized information prompts, and other management methods, carry out self-management behaviors and blood sugar control of patients with type 2 diabetes. An intervention study to explore the feasibility of mobile electronic technology in patients with type 2 diabetes, and to evaluate the impact of E-health interventions on patients' self-management behavior and blood sugar control.

纳入标准：

Inclusion criteria

排除标准：

①研究对象有严重的糖尿病并发症，例如糖尿病酮症酸中毒、糖尿病肾病等。
②研究对象有认知、听力、视力功能障碍，或患有严重的精神障碍或精神疾病。
③研究对象处于妊娠期或患有恶性肿瘤。
④研究对象参加过同类型的其他研究。

Exclusion criteria：

1. The research subjects have severe complications of diabetes, such as diabetic ketoacidosis, diabetic nephropathy, etc.; 2. The research subjects have cognitive, hearing, or a visual impairment, or suffer from severe mental disorders or mental illnesses; 3. The research object is pregnant or suffering from a malignant tumor; 4. The research subjects have participated in other studies of the same type.
