研究目的：

评估不同计算方法给出的胰岛素剂量对使用闭环的1型糖尿病患者的平均血糖达标时间和低血糖风险的影响。  

Objectives of Study：

To evaluate the safety and efficacy of different insulin dosing algorithm on the mean time in range and the risk of hypoglycemia in patients with type 1 diabetes treated with a closed-loop insulin pump.

药物成份或治疗方案详述：

经筛选且符合入选排除标准的受试者，将被按1:1的概率随机分到A组（先碳水化合物计数法，后脂肪蛋白质单位法）和B组（先脂肪蛋白质单位法，后碳水化合物计数法），并根据指定的顺序依次进行相应方案的治疗。具体治疗方案如下：导入期14天，佩戴CGM，优化胰岛素剂量、碳水化合物系数、胰岛素敏感系数；临床营养师对受试者进行碳水化合物计数法及改良脂肪蛋白质单位法进行一对一指导；再分组完成餐时不同算法对血糖控制和低血糖风险影响的研究。试验期间，受试者每日进行不少于3次的指血血糖监测。 

Description for medicine or protocol of treatment in detail：

The subjects will be randomly assigned to group A (First, carbohydrate counting followed by fat-protein unit) and group B (fat-protein unit followed by carbohydrate counting) with a probability of 1:1. Before randomization, during a run-in phase(14 days), each participant was trained to use the study pump and the continuous glucose monitor. The insulin dose, insulin carbohydrate ratio, and insulin sensitivity factor were optimized during the period, and the nutritionist guided carbohydrate counting and modified fat-protein unit. The effects of different meal algorithms on glycemic control and the risk of hypoglycemia were studied. During the trial, the subjects' finger blood glucose was monitored no less than three times daily. 

纳入标准：

Inclusion criteria

排除标准：

1.近6月发生过严重低血糖事件（Ⅲ级低血糖：没有特定血糖界限，伴有意识和/或躯体改变的严重事件，需要他人帮助的低血糖）或发生过酮症酸中毒，或服用非胰岛素抗高血糖药物者；2.有胃轻瘫、显著的糖尿病并发症（肾小球滤过率 < 45 mL/min或已经在透析的患者）;3.近期急性大血管事件

Exclusion criteria：

1. Severe hypoglycemia events (grade ⅲ hypoglycemia: severe events without specific blood glucose boundaries, accompanied by changes in consciousness or body, hypoglycemia requiring help from others), ketoacidosis, or non-insulin anti-hyperglycemia drugs in the past six months; 2. Patients with gastroparesis, significant diabetic complications (glomerular filtration rate < 45 mL/min or already on dialysis); 3. Recent acute macrovascular events.
