研究目的：

本实验旨在通过膈肌超声相关评估指标，量化术中对呼吸力学的积极因素，比较全麻期间VCV+5cmH2O PEEP（V1组）、PCV+5cmH2O PEEP（P1组）、VCV+个体化PEEP（V2组）， PCV+个体化PEEP（P2组）四种通气模式对下腹部腔镜手术患者短期膈肌功能的影响，寻找最为恰当的通气模式和呼吸参数来实施膈肌保护性通气。  

Objectives of Study：

The purpose of this study was to quantify the positive factors of respiratory mechanics during diaphragm ultrasound evaluation, and to compare the effects of VCV+5cmH2O PEEP (group V1), PCV+5cmH2O PEEP (group P1), VCV+ individualized PEEP (group V2) and PCV+ individualized PEEP (group P2) on short-term diaphragm function in patients undergoing lower abdominal endoscopic surgery, and to find the most appropriate ventilation mode and respiratory parameters to implement diaphragm protective ventilation.

纳入标准：

Inclusion criteria

排除标准：

1. 术前精神障碍、术前病态肥胖（BMI> 30 kg/m2），合并有慢性心肺疾病，缺氧（PaO 2< 60 mmHg或SPO2< 90%）、肺部感染；
2. 手术类型转换为剖腹手术、术中输血、CO2气腹持续时间<60分钟或>180分钟、膈肌超声成像不清晰、术后VAS 评分>3、术中出现其他不良事件。

Exclusion criteria：

1. Preoperative mental disorder, preoperative morbid obesity (BMI > 30 kg/m2), combined with chronic cardiopulmonary disease, hypoxia (PaO2 < 60 mmHg or SPO2 < 90%), lung infection;
2. The type of surgery was converted to laparotomy, intraoperative blood transfusion, duration of CO2 pneumoperitoneum < 60 minutes or > 180 minutes, unclear ultrasound imaging of the diaphragm, postoperative VAS score > 3, and other adverse events during the operation.
