研究目的：

针对认知功能障碍的老年住院患者，制定运动、认知双重任务型训练为主的干预方案，结合持续2年的随访、管理，探索“筛查-多重干预-联合管理”的老年多学科决策模式对老年认知功能障碍干预的远期效果，提高老年认知功能障碍的综合防控水平。

Objectives of Study：

To assess the long-term effects of simultaneous physical exercise and cognitive training based on multidisciplinary decision-making model in elderly inpatient with cognition impairment. The model of "screening-multiple interventions-combined management" is implemented, and all enrolled patients are followed up for 2 years.

纳入标准：

Inclusion criteria

排除标准：

1.精神、意识障碍； 2.严重视力或听力下降影响交流； 3.疾病终末期； 4.重要脏器功能严重衰竭； 5.急性重症患者；急性心肌梗死；剥离性主动脉瘤；重度主动脉瓣狭窄；心内膜炎/急性心包膜炎；急性肺栓塞或深静脉血栓；近期大手术或过去1个月发生肢体骨折者； 6.患者本人及家属拒绝参与本研究； 7.患者无法配合完成评估与干预； 8.存在研究者认为需排除的其他状况。

Exclusion criteria：

1. Mental or consciousness disorders, inability to communicate effectively; 2. Severe vision or hearing loss, inability to cooperate with evaluation or intervention; 3. Endstage disease; 4. Serious failure of vital organs; 5. Acute myocardial infarction; dissection aortic aneurysm; severe aortic valve stenosis; endocarditis /acute pericarditis; acute pulmonary embolism or deep vein thrombosis; recent major surgery or limb fracture in the past 1 month; 6. Patient or their family refuse to participate in the study; 7. Unable to coordinate the completion of assessment and intervention; 8. Other conditions that the researcher consider necessary to exclude.

应用区组分层随机法进行随机分组，分层因素为：认知功能障碍程度、受教育程度；分别设置为2个水平。由统计学专业人员应用随机数字表产生随机序列，符合条件的老年患者按1:1的比例随机分配到对照组或干预组。

Randomization Procedure (please state who generates the random number sequence and by what method)：

The stratified blocked randomization is used to randomize groups, and the stratified factors are degree of cognitive dysfunction and education level, divided into two levels respectively. Statistical professionals generate random sequences with a random number table, and eligible elderly patients are randomly allocated into test group or control group.
