研究目的：

探索互联网+模式在DR患者延续护理的构建研究，旨在利用信息化服务平台，建立医院-家庭动态化、突出中医特色的延续护理服务，以期提高患者疾病认知及自我管理能力，改善低视力患者生活质量，提升整体护理水平。  

Objectives of Study：

To explore Internet+ patterns in patients with DR continuation of the construction of nursing research, and to use information service platform, set up the hospital-the continuation of family dynamic, highlight the characteristics of TCM nursing service, in order to improve the patient's disease cognition and self management ability, improve the quality of life in patients with low vision, improve the overall nursing level.

纳入标准：

Inclusion criteria

排除标准：

1.合并有其他眼部疾病：视神经萎缩、虹膜睫状体炎、葡萄膜炎、黄斑变性、青光眼；
2.合并严重的心功能衰竭、其它严重的躯体疾病，生活不能自理的患者；
3.有认知障碍及沟通障碍者；
4.经门诊治疗未住院治疗者；
5.不同意复查者。

Exclusion criteria：

1. Associated with other eye diseases: optic atrophy, iridocyclitis, uveitis, macular degeneration, glaucoma;
2. Patients with severe heart failure or other serious physical diseases who are unable to take care of themselves;
3. People with cognitive and communication disabilities;
4. Outpatient treatment without hospitalization;
5. Those who disagree with the review.

对照组出院时即按照常规护理干预进行出院指导及随访，告知患者遵医嘱定期医院复查，如出现视力下降、眼睛不舒适则随时回院复查。

进行互联网+延续护理模式干预，包括服务系统的维护和服务资料的维护。系统维护由小组信息科、眼底病科护理人员负责，包括日常系统维护、患者答疑、存在问题收集与改进、统计后台相关数据（如推送链接点击次数、患者的提问等）；服务资料维护由眼底病科临床专家、眼底病科护理人员负责，以文字、图片、视频等形式，进行资料的更新与推送，内容包括健康教育知识，涉及防控、诊断、治疗、生活、康复、锻炼、护理、政策等。
