研究目的：

1.通过流行病学的方法，调查北京和台北市老年人照护需求特征及规律； 2.描述基于智慧社区的老人照护服务机构及相互间的激励和协同机制； 3.老人照护模式干预规则的设计与建立 4.建立SMART模式，并验证SMART模式对促进老人整体照护的效果。

Objectives of Study：

1. To investigate characteristics and patterns of the care needed by elderly with various conditions in Beijing and Taipei city, with epidemiological survey method; 2. To describe the underlining incentive and coordinative mechanism among different systems and facilities that provide or possibly provide health and social services to the elderly population who lived in smart community; 3. To design and construct the Rules of care model intervention for elderly people; 4. To develop an intelligent and integrated elderly care model (SMART), and evaluate the effect of SMART model in promoting Holistic care of elderly people.
