研究目的：

1.评价远程多学科延续护理对脑卒中居家康复患者日常生活活动能力（基础性日常生活活动能力和工具性日常生活活动能力）、运动功能（肌力和步行能力）的作用； 2.评价远程多学科延续护理对脑卒中居家康复患者卒中知识行为、6分钟步行距离、吞咽功能、跌倒、再入院率的作用。

Objectives of Study：

1. To evaluate the effect of remote multidisciplinary continuous care on activities of daily living (basic activities of daily living and instrumental activities of daily living) and motor functions (muscle strength and walking ability) of stroke patients undergoing home rehabilitation; 2. To evaluate the effect of remote multidisciplinary continuous nursing on stroke knowledge and behavior, 6-minute walking distance, swallowing function, fall and readmission rate in stroke patients undergoing home rehabilitation.

纳入标准：

Inclusion criteria

排除标准：

1.患有严重心肺、肝、肾功能不全或其他恶性疾病者；
2.有抑郁、痴呆病史或其他精神疾病者。

Exclusion criteria：

1. Those with severe heart and lung, liver, kidney insufficiency or other malignant diseases; 2. Those with a history of depression, dementia or other mental illnesses.
