研究目的：

本次研究希望通过开展医护一体化的血管通路门诊在AVF术后延续性护理中的应用，帮助医生护士全面的掌握患者病情，实现对AVF术后患者的系统化管理和个性化护理，提高医疗护理质量，完成随访数据库的建立以供临床科研和教学进一步使用。同时也能够满足患者更高层次的医疗护理需求，为其他延续性护理工作提供借鉴和参考。

Objectives of Study：

This study hopes to help doctors and nurses to fully grasp the patient's condition by carrying out the application of the vascular access clinic integrating medical care and nursing in the continuous care after AVF, realize the systematic management and personalized care of patients after AVF, and improve the quality of medical care. Nursing quality, complete the establishment of follow-up database for further use in clinical research and teaching. At the same time, it can also meet the higher level of medical care needs of patients, and provide reference and reference for other continuous nursing work.

Inclusion criteria

排除标准：

1. 有严重心脑血管疾病；
2. 合并自身免疫性、感染性疾病；
3. 合并系统性皮肤疾病；
4. 合并恶性肿瘤；
5. 沟通障碍，无法配合研究。

Exclusion criteria：

1. Severe cardiovascular and cerebrovascular diseases;
2. Combined with autoimmune and infectious diseases;
3. Combined with systemic skin diseases;
4. Combined with malignant tumors;
5. Communication barriers, unable to cooperate with research.
