研究目的：

本研究希望将微信平台结合个体化营养管理应用于PD患者当中，通过PD护理专科门诊为患者制定具体可行的个体化营养管理方案，并借助微信对患者进行有效监督和指导，改变患者营养管理观念薄弱、居家营养管理依从性低的现状，提高患者肾脏病饮食态度和行为依从性，改善患者的营养状态和生活质量。同时总结该方式的应用效果，为临床营养管理工作提供借鉴和参考。  

Objectives of Study：

This study hopes to apply the WeChat platform combined with individualized nutrition management to PD patients, formulate specific and feasible individualized nutrition management plans for patients through the PD nursing clinic, and use WeChat to effectively supervise and guide patients, so as to change the concept of nutrition management for patients weakness and low compliance with home nutrition management, improve the dietary attitude and behavioral compliance of patients with kidney disease, and improve the nutritional status and quality of life of patients. At the same time, the application effect of this method is summarized to provide reference and reference for clinical nutrition management.

Inclusion criteria

排除标准：

1. 有严重心脑血管疾病；
2. 合并自身免疫性、感染性疾病；
3. 合并系统性皮肤疾病；
4. 合并恶性肿瘤；
5. 沟通障碍，无法配合研究。

Exclusion criteria：

1. Severe cardiovascular and cerebrovascular diseases;
2. Combined with autoimmune and infectious diseases;
3. Combined with systemic skin diseases;
4. Combined with malignant tumors;
5. Communication barriers, unable to cooperate with research.
