Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

We will keep the information confidential for the subjects. The names of subjects will be coded as study ID. All the questionnaires will be stored for regularly checked and rechecked by data monitoring committee. Data monitoring committee is independent from the sponsor and competing interests. All data will be extracted and calculated twice to ensure the accuracy by trained researchers.
