研究目的:

(1)从生物力学的角度观察针灸后膝内侧间室骨性关节炎患者登梯运动功能的变化,包括关节负重的运动学和动力学的变化,关节内应力应变的分布情况,膝关节周围组织结构在力学方面的变化等; (2)分析步态分析的客观指标与主观结局评测指标(WOMAC)之间的相关性; (3)分析与针灸促进膝内侧间室骨性关节炎患者运动功能康复效应的相关生物力学因素,如各关节和膝屈伸肌群活动时力矩的变化,主动肌-拮抗肌之间的协调性、下肢力线的改变,关节面接触点的变化,关节内应力应变分布情况的变化,软骨、韧带在登梯时的应力情况等; (3) 为临床应用此生物力学方法评价膝内侧间室骨性关节炎康复疗效提供客观依据。  

Objectives of Study:

(1) To investigate the biomechanical effect of acupuncture on stair climbing by gait analysis in knee osteoarthiritis (KOA), including the changes of kinematics and kinetics, as well as the loading distribution in the knee joint; (2) To explore the relationship between objective assessment (gait analysis and surface EMG) and subjective measurement (WOMAC). (3)To analyze the possible mechanism underlying the effect of acupuncture on movement in KOA from biomechanical perspective, including changes of muscle mechanics, loading in the joints and so on, during stair climbing. (4) To provide the objective basis for clinical application of gait analysis as outcome measurement for KOA.

纳入标准:

Inclusion criteria

排除标准:

①心、肝、肾功能不全,严重结核病,急性化脓性、传染性病变及慢性皮肤病,动脉压降低者。
②其他神经肌肉骨骼系统的疾病影响步态,和/或认知障碍者,如类风湿性关节炎患者,神经源性关节病变,中风及骨折患者。
③Kellgren/Lawrence(K/L)分级为4级,关节严重畸形,无法独立行走者。
④近2个月内曾行关节内类固醇注射。
⑤在一年内准备进行全膝关节置换术。
⑥近1周内曾接受过药物治疗,现代或中医康复方法者。
⑦惧怕针灸无法配合治疗者。
符合以上任何一条标准者,不予纳入研究。

Exclusion criteria:

(1) lateral compartment KOA;
(2) asymptomatic KOA patients or other neuromusculoskeletal diseases that may affect gait and/or cognitive function, such as lower extremity injury, rheumatoid arthritis, neuropathic arthropathy, stroke or fracture;
(3) accepted intra-articular injection such as corticosteroid in the previous two months;
(4) had accepted a total knee replacement or plan to take the surgery in the following year;
(5) underwent any biomedical treatment such as medication, exercise program or physical therapy, TCM and other rehabilitative therapy within a week.

由统计人员先依次编写好1~XX的数字顺序表,通过SAS统计软件的PROC PLAN过程编程产生随机数字。将合格且已完成为期2周的等待处置的受试者按照入选的先后顺序按照各组1:1的比例随机分组,可预先设定产生的随机数字范围为A、B两组,根据随机数字范围将受试者分配至A组或B组,运行SAS统计软件的PROC PLAN过程生成随机数字表,将程序产生的随机分组结果打印出来,制订编码。

盲法:

本研究无法对施针者设置盲法,采用单盲法对研究对象设置盲法,即用字母A、B代替研究报告表中的组别。结局评价人员、数据管理和统计分析人员均由不参与治疗的第三方人员进行。通过SAS9.1软件设置盲底,并由项目管理员保存。

Blinding:

Acupuncture can't be carried through blinding method. So only single blinding is used for the subjects. Letter of A or B stands for either of the two groups. Outcome measure, data management and statistics is done by the third party person. Software of SAS 9.1 is used to setup blinding results, which are kept by project coordinator.
