研究目的：

我们课题拟应用“互联网+”信息技术手段，探索出一种针对2型糖尿病出院患者的新型药学随访模式，以期提高患者用药依从性、实现个体化药学服务。  

Objectives of Study：

Our project intends to use "Internet +" information technology to explore a new pharmaceutical follow-up model for discharged patients with type 2 diabetes, in order to improve patients' medication compliance and realize personalized pharmaceutical care.

纳入标准：

Inclusion criteria

排除标准：

1. 任何非2型糖尿病患者，如T1DM、妊娠糖尿病等； 2. 言语交流困难，不能很好地与临床药师沟通的患者； 3. 依从性差、抗拒检查的患者； 4. 严重认知障碍、缺乏生活自理能力； 5. 同时参与其他临床试验的患者； 还包括除上述外等不适合纳入研究的情况。

Exclusion criteria：

1. Any patients without type 2 diabetes, such as T1DM, gestational diabetes, etc.; 2. Patients who have difficulty in verbal communication and cannot communicate well with clinical pharmacists; 3. Patients with poor compliance and resistance to examination; 4. Severe cognitive impairment, lack of self-care ability; 5. Patients participating in other clinical trials at the same time; It also includes circumstances other than those mentioned above that are not suitable for inclusion in the study.

本研究使用来自研究随机化程序(https://www.randomizer.org/)的计算机生成的随机列表进行随机化，生成1组1000个含“0”和“1”的数字集合，根据患者入组的时间顺次在随机数表找到数集中的数字，该数字一经使用不再放回。根据所在位置“0”和“1”的数值，将分配到“0”的患者分配到对照组，将分配到“1”的患者分配到干预组。如遇同时需多个患者入组的情况，则请第三方裁定。（第三方预先不知道入组患者基本情况）
