研究目的：

1. 调查轻度认知障碍患者合并糖尿病自我管理水平现状并进行亚群分类； 2. 构建轻度认知功能障碍合并糖尿病患者自我管理分层支持模式； 3. 评价轻度认知功能障碍合并糖尿病患者自我管理分层支持模式的效果。  

Objectives of Study：

1. To investigate the self-management level of mild cognitive impairment patients with diabetes and classify the subgroups; 2. To construct a self-management tiered support model for patients with mild cognitive impairment and diabetes; 3. To evaluate the effect of self-management tiered support model in patients with mild cognitive impairment and diabetes.

纳入标准：

Inclusion criteria

排除标准：

1. 严重躯体疾病，无法完成认知功能筛查者；
2. 有抑郁、焦虑等精神疾病病史者；
3. 药物或酒精依赖症患者；
4. 存在其他可引起脑功能障碍的神经系统疾病和严重内科疾病的患者。

Exclusion criteria：

1. Those with severe physical diseases who cannot complete the cognitive function screening;
2. Those with a history of mental illness such as depression and anxiety;
3. Drug or alcohol dependent patients;
4. Patients with other neurological diseases and serious medical diseases that can cause brain dysfunction.
