研究目的:

通过对脑卒中患者的患侧关键肌的肌电具体改变,横向分析不同组别患者上肢康复疗效,再纵向对比患者自身康复前后的差异,以期量化MMT、FMA与肌电改变的相关性,获得肌电-肌力的线性关系、关联指标。为脑卒中患者提供精准康复评定及更有针对性的康复疗法,也为电生理EMG融入康复医学提供了新的理论基础。  

Objectives of Study:

Through the specific changes of EMG in the key muscles of stroke patients’ affected side, the curative effect of upper limb rehabilitation in different groups of patients will be analyzed and the differences before and after the rehabilitation of patients will be compared as well. In order to quantify the correlation between MMT, FMA and EMG changes, and obtain the linear relationship and correlation index of EMG-muscle strength. The study will provide a accurate rehabilitation assessment method and more targeted rehabilitation therapy for stroke patients, and also provides a new theoretical basis for the integration of EMG into rehabilitation medicine.

药物成份或治疗方案详述:

本研究针对脑卒中患者展开,患者分两组分别实施常规康复+上肢机器人训练,以及常规康复+功能性电刺激联合上肢机器人训练。在康复前后用MMT、FMA-UE评估患侧肢体功能,并选取患侧上肢关键肌(肱二头肌、肱三头肌、桡侧腕屈肌、尺侧腕屈肌、第一背侧骨间肌、拇短展肌)测得其表面肌电图(surface electromyography,sEMG)以及针极肌电图(needle electromyography,nEMG)的数据,评价上肢功能及肌力的进展,并分析康复过程中随肌力、功能变化后肌纤维募集模式的改变。 

Description for medicine or protocol of treatment in detail:

In this study, stroke patients will be divided into two groups. Patients in control group will receive conventional rehabilitation and upper limb robot training and patient in experimental group will receive conventional rehabilitation and functional electrical stimulation combined with upper limb robot training. MMT and FMA-UE will be used to evaluate the limb function of the affected side before and after rehabilitation. The key muscles of the affected upper limb (biceps brachii, triceps brachii, flexor carpi radialis, flexor carpi ulnaris, first dorsal interosseous muscle and abductor pollicis breis) will be tested by sEMG and nEMG, which can evaluate the progress of upper limb function and muscle strength. And then the data of the muscle fiber recruitment patterns varied with the change of muscle strength and function during rehabilitation will be analyzed. 

纳入标准:

Inclusion criteria

排除标准:

1.非保守治疗,或有颅骨缺陷者;
2.合并帕金森病或其他神经系统疾病者;
3.心、肺等脏器有严重疾病的患者;
4.既往有精神病史、药物滥用及酗酒史者;
5.病情不稳定的患者,如血压指标:低压≤90mmHg或高压≥150mmHg;
6.凝血功能障碍患者;
7.严重视、听觉、认知及交流障碍而不能进行训练者。

Exclusion criteria:

1. Non-conservative treatment, or skull defects;
2. Patients with Parkinson's disease or other neurological diseases;
3. Patients with serious diseases of the heart, lungs and other organs;
4. Previous history of mental illness, drug abuse and alcohol abuse;
5. Patients with the unstable condition, such as blood pressure indicators: low pressure <= 90mmHg or high pressure >= 150mmHg;
6. Patients with coagulation dysfunction;
7. Strict disorder of attention, hearing, cognition, and communication who can not be trained.

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

The Case Record Form (CRF) will be used to record key clinical data, adverse events, and safety assessments during the study period, with each participant having a unique numeric identifier. Clinical research assistants will verify the accuracy, omissions, and consistency of data in the CRF. Clinical questionnaires will be distributed to all participants in a separate room and conducted by the same researchers. Experimental data will be independently entered into the electronic database by two researchers, and when inconsistent data appears, it will be tested with reference to the original data source. The electronic data will be exported to Microsoft Excel and then analyzed using the SPSS software package (SPSS 25.0).
