研究目的:

利用现有的电讯及互联网资源,构建基于“互联网+”技术的健康教育新模式的健康教育平台,探讨“互联网+健康教育”新模式在重庆市家庭医生签约服务中对T2DM患者管理的效果。通过搭建家庭医生团队和签约家庭之间的网络交流平台,降低家庭医生团队工作时间成本,提高家庭医生团队的工作效率,有效改善家庭医生团队开展社区健康教育工作中存在的问题。  

Objectives of Study:

Using the existing telecommunications and Internet resources, this paper constructs a health education platform of the new model of health education based on "Internet +" technology, and discusses the effect of the new model of "Internet + health education" on the management of T2DM patients in the contract service of family doctors in Chongqing. By building a network communication platform between the family doctor team and the contracted family, we can reduce the working time cost of the family doctor team, improve the work efficiency of the family doctor team, and effectively improve the problems existing in the community health education work of the family doctor team.

Inclusion criteria

排除标准:

(1)存在严重的并发症,如酮症酸中毒、高渗性非酮症糖尿病昏迷等患者。 (2)存在严重的疾病,如肿瘤、急性心脑血管病、胰腺炎等;伴有严重心脏疾病,不能耐受中等以上强度运动的患者。 (3)不能进行行为干预的个体 (不能清晰回答问题、不会使用微信、正在参加其他类似研究)。 (4)拒绝参与干预研究。

Exclusion criteria:

(1) Patients with severe complications, such as ketoacidosis, hyperosmolar non-ketotic diabetic coma, etc. (2) Patients with serious diseases, such as tumor, acute cardio-cerebrovascular disease, pancreatitis, etc. Patients with severe heart disease who cannot tolerate moderate intensity exercise. (3) Individuals who cannot perform behavioral intervention (cannot answer questions clearly, do not use WeChat, and are participating in other similar studies). (4) Refuse to participate in intervention research.

干预组的参与者被邀请到微信组,在健康教育网络平台上,连续完成一个糖尿病健康管理培训。健康管理培训包括两个阶段:学习糖尿病健康教育知识和养成健康行为。 第一阶段:以微信公众号“巴渝健康”为平台,参加糖尿病在线课程,系统学习健康生活方式、血糖监测、用药指导、并发症管理等自我管理知识和技能。糖尿病课程由军队健康教育中心根据《中国糖尿病基础控制与管理手册(2019)》设计。糖尿病课程包括30个短视频,对应视频的30个问题,对应视频的30篇短文,以及特殊教育(关于睡眠、运动和饮食)。参与者需要观看15分钟的视频,课后回答问题,分享健康小贴士,并参与讨论。健康助理会进行每日总结,表扬这些优秀的学习者,并在微信组上通知下一个学习计划。 第二阶段:以“微步有约”小程序为基础,加入家庭医生圈,参与糖尿病自我管理健康活动,包括自我管理测试、设定健康管理目标、执行日常任务。糖尿病自我管理目标包括16个项目:积极随访、定期随访、积极就医、处方用药、定期用药、严格饮食、甜点摄入、食物选择、暴饮暴食、自我管理、血糖监测、血糖水平、血糖记录、运动计划、定期运动、积极运动。在健康活动圈,参与者还被要求用照片/图片分享他们的每日步数、每日饮食摄入量、身体成分数据和血糖监测。每次参与者完成一项在线活动,系统都会给予一些心形作为奖励。红心可以换取相应的礼物,激励患者培养健康的生活习惯。这些帖子只能被参与者和糖尿病健康教育团队查看,这有助于家庭医生对这些糖尿病患者进行监督和管理。

干预措施代码:

Intervention:

Participants in the intervention group were invited into the WeChat group to complete a diabetes health management training program in succession, based on the health education network platform. The health management training program consisted of two stages: Learn diabetes health education knowledge and develop healthy behaviors . The first stage: Based on the WeChat public account “Bayu Health”, participants taken an online diabetes course to learn the knowledge and skills of self-management systematically, including healthy lifestyle, blood glucose monitoring, medication guidance, and management of complications. Diabetes course was designed by Military Health Education Center, according to the National Basic Diabetes Control and Management Manual (2019) in China. The diabetes course includes 30 short videos, 30 questions for the corresponding videos, 30 short articles for the corresponding videos, and special education (about sleep, exercise and diet). Participants needed to watch 15-minutes video, answer questions after class, share tips on health, and participate in discussions. The health assistant would conduct a daily summary, praise these good learners, and inform the next study plan on the WeChat group. The second stage: Based on the “WeWalk” mini program, participants would join the family doctors circle to participate in diabetes self-management health activities including self-management test, health management goals set, daily tasks implementation. The goals of diabetes self-management included 16 items: active follow-up, regular follow-up, active medical treatment, prescribed medication, regular medication, strict diet, dessert intake, food choice, overeating, self-management, blood glucose monitoring, blood glucose level, blood glucose recording, exercise plan, regular exercise and active exercise. In the health activity circle, participants were also instructed to share their daily steps, daily dietary intake, body composition data, and blood glucose monitoring with photos/pictures. Each time participant completed an online activity, the system would give some hearts as a reward. The red heart can be exchanged for corresponding gifts to motivate patients to cultivate the healthy habits. These posts can only be viewed by the participants and diabetes health education team, which helped family doctors to supervise and manage these diabetes patients.
