研究目的：

通过临床征集Wagner3~4级糖尿病足溃疡患者，采用TTT技术联合神经松解术对患者患肢进行处理，观察其对Wagner3~4级糖足创面治疗的临床效果，探索一种能够有效治疗难治性糖尿病足的新方法。

Objectives of Study：

To collect Wagner grade 3~4 diabetes foot ulcer patients, use TTT technology combined with neurolysis to treat the affected limbs, observe its clinical effect on Wagner grade 3~4 diabetic foot wound, and explore a new method that can effectively treat refractory diabetes foot.

纳入标准：

Inclusion criteria

排除标准：

1.患肢腘动脉完全阻塞，无侧支循环建立； 2.静脉栓塞、淋巴管炎等动静脉血管性病变患者； 3.心、肺、肝、肾等脏器功能严重不全患者； 4.糖尿病急性并发症，如糖尿病高渗性昏迷、糖尿病酮症及酮症酸中毒、乳酸性酸中毒等； 5.合并恶性肿瘤者； 6.孕妇或哺乳期妇女。

Exclusion criteria：

1. The popliteal artery of the affected limb was completely blocked and no collateral circulation was established; 2. Patients with arteriovenous vascular diseases such as venous embolism and lymphangitis; 3. Patients with severe dysfunction of heart, lung, liver, kidney and other organs; 4. Acute complications of diabetes, such as hypertonic coma of diabetes, ketosis and ketoacidosis of diabetes, lactic acidosis, etc; 5. Patients with malignant tumor; 6. Pregnant or lactating women.

本试验为探索性验证，确定每组的样本量为30例，试验分两组进行，单纯TTT治疗组和TTT联合神经松解术治疗组，共需样本量60例。病例组与对照组的分配采用1:1均衡设计。研究者对符合入选、不符合排除标准的患者根据先后顺序，随机编号分组。

Randomization Procedure (please state who generates the random number sequence and by what method)：

This test is exploratory verification, and the sample size of each group is determined to be 30 cases. The test is divided into two groups, the TTT treatment group and the TTT combined neurolysis treatment group, with a total sample size of 60 cases. A 1:1 balanced design was used for the allocation of cases to controls. Patients who met the inclusion criteria and those who did not meet the exclusion criteria were assigned to each group by random number, according to the order of enrollment.
