研究目的：

本项目拟对老年2型糖尿病人群（60岁及以上），进行传统老年综合评估及interRAI-HC评估，探讨基于interRAI-HC在老年2型糖尿病中评估对发现认知障碍及身体功能障碍与传统表单评估结果的差异；探讨进一步针对评估结果采取干预措施对老年2型糖尿病中合并认知障碍的改善，进而提出老年2型糖尿病患者居家管理策略。

Objectives of Study：

This project intends to conduct traditional comprehensive geriatric assessment and Interrai-HC assessment in elderly patients with type 2 diabetes (aged 60 years and above), to explore the differences between the findings of cognitive impairment and physical dysfunction in elderly patients with type 2 diabetes based on interrai-HC assessment and traditional form assessment. To explore the improvement of cognitive impairment in elderly patients with type 2 diabetes mellitus by further intervention measures based on the evaluation results, and then put forward home management strategies for elderly patients with type 2 diabetes mellitus.

纳入标准：

Inclusion criteria

排除标准：

1. 伴有糖尿病急性并发症如严重感染、糖尿病酮症酸中毒、高渗高血糖综合征等；
2. 既往有脑血管意外事件，遗留血管性认知障碍；
3. 严重认知、视力、听力、语言障碍等无法完成调查表中的内容；
4. 终末期患者（预期寿命不足半年）；
5. 研究者认为不适合参与本研究的其他情况。

Exclusion criteria：

1. Accompanied by acute complications of diabetes such as severe infection, diabetic ketoacidosis, hyperosmolar hyperglycemia syndrome, etc.;
2. Previous cerebrovascular accidents and vascular cognitive impairment;
3. Severe cognitive, vision, hearing, language impairments, etc. cannot complete the contents of the questionnaire;
4. End-stage patients (life expectancy less than half a year);
5. Other conditions that the researcher thinks are not suitable for participating in this study.
