研究目的：

1.调查社区常见老年慢性病患者居家用药安全知信行的现况； 2.构建出适合社区老年慢性病患者的用药安全健康教育内容； 3.比较常规健康教育、回授法健康教育、家庭参与式健康教育及家庭参与式的回授法健康教育4种教育模式对社区老年慢性病患者用药安全的作用效果。

Objectives of Study：

1.Investigate the current status of knowledge, belief, and practice of home medication safety among elderly patients with chronic diseases in the community; 2.Constructing medication safety and health education content suitable for elderly patients with chronic diseases in the community; 3.To compare the effect of four educational modes: conventional health education, teaching-back health education, family-participating health education, and family-participating teaching-back health education on drug safety of elderly patients with chronic diseases in the community.

排除标准：

患者：
听力障碍、言语障碍、认知障碍、精神疾病、重症或终末疾病的患者。
家庭照顾者：
1.存在认知或精神障碍者；
2.行动不便者。

Exclusion criteria：

Patients:
Patients with hearing impairment, speech impairment, cognitive impairment, mental illness, severe or terminal illness.
Family caregivers:
1. Individuals with cognitive or mental disorders;
2. Those with limited mobility.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：	由经过专门培训的资料收集员对数据进行测量和采集。采用Excel 2016建立数据库，为保证数据录入的准确性，所有数据均双人录入；采用SPSS软件进行数据处理。
