研究目的：

1.描述卒中患者出院后体力活动状况及疲劳状况。 2.分析卒中患者体力活动水平与卒中后疲劳的关系及卒中后疲劳的影响因素。 3.评价基于微信平台的延续性护理对卒中后疲劳的干预效果。  

Objectives of Study：

1. Describe the physical activity status and fatigue status of stroke patients after discharge; 2. To analyze the relationship between physical activity level and post-stroke fatigue in patients with stroke and the influencing factors of post-stroke fatigue; 3. Evaluate the intervention effect of continuous care based on wechat platform on post-stroke fatigue.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

Inclusion criteria

排除标准：

1.有脑卒中外的肢体功能障碍的疾病。
2.伴有严重的心、肝、肾功能不全、呼吸衰竭及恶性肿瘤的患者；
3.存在认知功能障碍；
4.伴有多发性硬化、帕金森病、贫血等已知疲劳高发疾病的患者。

Exclusion criteria：

1. There is a disease of limb dysfunction outside stroke.
2. Patients with severe heart, liver, renal insufficiency, respiratory failure and malignant tumor;
3. Cognitive dysfunction;
4. Patients with multiple sclerosis, Parkinson's disease, anemia and other known fatigue diseases.
