研究目的:

颅内动脉瘤是严重危害人类健康的主要疾病之一,目前最主要的治疗手段是外科夹闭和血管内栓塞。治疗后的颅内动脉瘤,特别是不全闭塞的动脉瘤,存在治疗后复发和再出血的风险。本研究包括颅内动脉瘤3D模型的建立及临床病例的研究,研究不同部位、大小和形态的颅内动脉瘤手术夹闭前,3D打印技术构建出动脉瘤3D模型,结合模型完成术前讨论,预估动脉瘤与毗邻骨质、血管、脑实质的解剖关系、动脉瘤的瘤体朝向,模拟瘤颈的分离操作、选择放置合适的动脉瘤夹,并将模型应用到术中供术者参考,从而为临床手术夹闭动脉瘤提供理论依据与实践基础。构建动脉瘤3D模型模拟指导手术的优势在于:①直观颅内动脉全貌,在手术过程中将3D模型置于手术室,多方位、多角度旋转,可了解动脉瘤患者脑血管解剖特点,评估术中重要血管、脑实质、颅骨解剖关系,明确动脉瘤的瘤体朝向,并可模拟瘤颈分离操作,减少术中不必要的解剖显露,降低动脉瘤破裂出血发生率;②传统动脉瘤夹闭术大部是通过翼点入路进行手术,对于颅内复杂动脉瘤,由于视角单一可能造成动脉瘤显露不完全,直接导致夹闭不全、残余等情况,应用3D模型术前模拟可降低此类事件发生率;③通过3D模型多方位旋转视角还可了解动脉瘤背侧情况以及复杂动脉瘤与穿支动脉的解剖关系。此外还可应用模型做术前讨论以及对青年医生进行培训,同时方便患者及家属可以详细直观了解病情,正确认识手术风险,从而形成良好的医患沟通。  

Objectives of Study:

Intracranial aneurysm is one of the main diseases that seriously endanger human health. At present, the main treatment is surgical clipping and endovascular embolization. Treatment of intracranial aneurysms, in particular, is not total occlusion of the aneurysm, there is a risk of recurrence and re bleeding after treatment. In this study, including research to establish 3D model of intracranial aneurysms and the clinical cases of intracranial aneurysm surgery, different position, size and shape of the clip, 3D printing technology to construct the aneurysm model combined with 3D model, completes the discussion of preoperative prediction of aneurysm and adjacent bone, blood vessels, brain anatomy, the aneurysm body towards the separation operation, selection of simulation of aneurysm neck placed suitable aneurysm clip, and the model is applied to the operation in the operation for reference, so as to provide a theoretical basis and practical basis for clinical surgery aneurysm clip. Is to construct simulation guide surgery aneurysm 3D model advantages: visualization of intracranial artery in the operation process of the picture, the 3D model in the operation room, multi-directional, multi angle rotation, can understand the aneurysms in patients with cerebral vascular anatomy, intraoperative evaluation of important blood vessels and brain parenchyma, skull anatomy, clear tumor toward the aneurysm, and can simulate the neck separation operation, reduce unnecessary intraoperative anatomic exposure, reduce the incidence of bleeding of ruptured aneurysms; the traditional aneurysm surgery department is surgery through pterional approach, for complex intracranial aneurysm, because the single perspective may be caused by the exposure of aneurysm is not complete, directly cause the clamp incomplete closure and residual etc., the application of 3D model simulation before operation can reduce the incidence of such events; through the 3D model of multi azimuth rotation angle can also understand the dorsal artery aneurysms As well as the complex aneurysms and perforating artery anatomic relationship. In addition, the model can also be used to discuss the operation of young doctors and training, while the patients and their families can be convenient and intuitive understanding of the disease, the correct understanding of the risk of surgery, so as to form a good communication between doctors and patients.

排除标准:

年龄小于18岁或大于80岁; Hunt-Hess分级4级以上;既往有脑卒中、脑外伤等神经系统损伤病史;有明确颅内局部血肿;动脉瘤直径超过15mm;为颅内多发动脉瘤

Exclusion criteria:

Has a history of stroke, brain trauma and other neurological history; clear intracranial hematoma; aneurysm diameter of more than 15mm; for multiple intracranial aneurysms
