研究目的：

本课题为多中心临床试验研究，采用非劣性、开放标签、随机对照的研究方法比较直肠癌开放性手术和腹腔镜手术的近期和远期疗效差异，并且评价不同腹腔镜手术术式的治疗效果。此外本课题以评分量表的形式对直肠癌术后功能性指标进行系统量化，并从免疫功能及应激反应等方面对两组疗效进行综合评估。

Objectives of Study：

The objective of this study was to compare the short-term and long-term outcomes of open and laparoscopic surgery with noninferiority, open-label, and randomized controlled trials, and to evaluate the efficacy of different laparoscopic procedures. In addition to the scale of the subject in the form of functional indicators of rectal cancer after the system quantification, and from the immune function and stress response to the two groups to conduct a comprehensive assessment of efficacy.

纳入标准：

Inclusion criteria

排除标准：

①ASA分级＞III级；
②存在气腹禁忌证（如严重心肺功能不全者）；
③5年内有盆腔恶性肿瘤病史者；
④急性肠梗阻的患者；
⑤满足经肛门局切标准的T1期直肠癌患者；
⑥既往多次行剖腹手术或腹腔广泛粘连者；
⑦家族性腺瘤性息肉病，林奇综合征相关性直肠癌以及炎症性肠病活动期的患者

Exclusion criteria：

1. ASA classification> Grade III;
2. Pneumoperitoneum contraindications (such as severe cardiopulmonary dysfunction);
3. The history of pelvic malignancy within 5 years ;
4. Acute intestinal obstruction;
5. meet the criteria of transanal excision of T1 rectal cancer patients;
6. underwent multiple laparotomy or abdominal adhesions widely;
7. familial adenomatous polyposis, Lynch syndrome-related rectal cancer and inflammatory bowel disease in patients with active period.
