研究目的:

1.本研究旨在通过探究良恶性肺结节患者血液的ctDNA甲基化水平、突变特征,以及外周血TCR特征,绘制恶性肺结节特异性甲基化图谱和突变图谱,以及恶性肺结节相关的TCR特征谱。进而通过先进的机器学习方法将ctDNA的甲基化和基因突变以及TCR特征数据,与传统的肿瘤标志物以及临床特征数据相结合,构建一个新的联合诊断模型,实现良性/恶性结节的有效区分,一方面,减少良性结节患者的侵入性诊断筛查和辐射暴露,避免过度诊断;另一方面,提高肺癌筛查的效率,发现更多的早期恶性病变,可以有效提高患者的治愈率,实现肺癌早诊。2.收集病人临床资料特征,通过组织标本及血液样本的全基因测序,验证其一致性,准确性,灵敏性及特异性;对肺癌术后病人,进行精准化及个体化治疗,通过定期液体活检,监测患者术后血液中TMB及ctDNA的动态变化,进而,有效监测患者疾病的进展情况,反映肺癌患者的预后情况,对患者进行全程化、精准化及个体化跟踪治疗,为患者选择合理的治疗方案,改善患者的预后,提高患者治疗效果。3.人才培养和学科建设:随着本项目的开展实施,将培养出一批有着丰富肺部结节临床诊治经验又具备液体活检研究经验的复合型人才,为个体化诊疗时代培养一批经验丰富基础扎实的接班人。  

Objectives of Study:

1.This study aims to draw the methylation map and mutation map of malignant pulmonary nodules, as well as the TCR characteristic spectrum related to malignant pulmonary nodules by exploring the ctDNA methylation level and mutation characteristics of blood and TCR characteristics of peripheral blood of patients with benign and malignant pulmonary nodules. And then through the use of advanced machine learning methods will ctDNA methylation and gene mutation, and which is the characteristic data, and the traditional combination of tumor markers, and clinical features of the data, to build a new joint diagnosis model, implement effective distinguish benign/malignant nodules, on the one hand, reducing invasive diagnostic screening in patients with benign nodules and radiation exposure, avoid excessive diagnosis; On the other hand, improving the efficiency of lung cancer screening and finding more early malignant lesions can effectively improve the cure rate of patients and achieve early diagnosis of lung cancer. 2.Collect clinical data of patients and verify their consistency, accuracy, sensitivity and specificity by whole-gene sequencing of tissue samples and blood samples; On postoperative lung cancer patients, accurate and individualized treatment, through regular liquid biopsy, and monitoring of patients with postoperative blood TMB and ctDNA dynamic changes, thus, effective monitoring of the progress of the disease patients, reflect the prognosis of the patients with lung cancer, the patients track whole-coursing, accurate and individualized treatment, treatment options for patients with a reasonable solution, Improve the prognosis of patients, improve the treatment effect of patients. 3.Talent cultivation and discipline construction: With the implementation of this project, a group of compound talents with rich clinical diagnosis and treatment experience of pulmonary nodules and liquid biopsy research experience will be cultivated, so as to train a group of experienced and well-grounded successors for the era of individualized diagnosis and treatment.

纳入标准:

Inclusion criteria

排除标准:

1.孕妇或哺乳期妇女;
2.肺结节直径大于30mm;
3.一名患者有2个或更多肺结节;
4.术后未病理确诊的患者,或入组前2年内病理确诊的癌症患者;
5.任何淋巴或远处转移的迹象;
6.研究者认为可影响方案依从性,或影响患者签署知情同意书(ICF),或不适宜参加本研究的具有临床意义的任何其它疾病或状况。

Exclusion criteria:

1.Pregnant or lactating women;
2.The diameter of pulmonary nodules is greater than 30mm;
3.One patient had 2 or more pulmonary nodules;
4.Patients who were not pathologically confirmed after surgery or patients with cancer pathologically confirmed within 2 years before enrollment;
5.Any signs of lymphatic or distant metastasis;
6.Any other disease or condition of clinical significance that the investigator considers may affect protocol compliance,or affect patients' signing of informed consent (ICF), or make them unsuitable for the study.
