研究目的：

本研究旨在比较三种不同材质的枕头在神经ICU患者中压力性损伤发生的关系，从而为临床工作者选取合适的枕头以预防ICU患者枕部压疮提供有力的依据。

Objectives of Study：

The purpose of this study was to compare the relationship between three different pillow materials and the occurrence of stress injury in ICU patients, so as to provide a strong basis for clinical workers to select appropriate pillows to prevent occipital pressure ulcers in ICU patients.

药物成份或治疗方案详述：

2 研究方法 2.1不同材质枕头分组护理本研究采用棉花枕、水枕和凝胶枕，棉花枕使医院统一配置，水枕和凝胶枕根据研究需要由科室自行配置。患者枕头选取时根据患者病情的严重程度及受伤部位，综合考虑，选取患者合适的枕头。在护理过程中，使用气垫床，每两小时翻身一次，每次翻身时护士均对病人进行全身皮肤情况检查，观察皮肤完整性。加强体位管理，左侧卧位、平卧位、右侧卧位交替进行。2.1.1 对照组棉花枕：采用的是河南郑州天使服装有限公司，医院统一采购、长70 cm、宽40 cm的普通长方形枕头，内芯：聚酯纤维60.5%和棉39.5%，符合GB 18401-2010《国家纺织产品基本安全技术规范》标准C类要求。 2.1.2实验组1（水枕组）采用医用橡胶冰袋（小树牌），大小为45cm×20.5cm，容量为3L），符合国家入市标准，大小为45cm×20.5cm，容量为3L。洗净浸泡消毒晾干后，装入自来水1500～2000 mL，水量以平放在桌面上以一拳力量压在保温袋上拳头的凸面不接触桌面为宜，排尽袋内空气，将进水口打结系牢，外面套双层棉质枕套。 2.1.3试验组2（凝胶枕组）采用日产GELTRON凝胶枕，符合国家标准，大小为60cm×33cm×5.5~7.5cm，重量为3kg，内部材质为凝胶、高透气性海绵和高密度性海绵，外罩材质为60%聚酯纤维、35%天丝以及5%聚氨酯。

2. Research Methods 2.1 Pillows of different materials are taken care of in groups Cotton pillows, water pillows and gel pillows were used in this study. Cotton pillows were uniformly configured in the hospital, and water pillows and gel pillows were configured by the department according to the research needs. Patient pillow selection according to the severity of the patient's condition and injury site, comprehensive consideration, select the appropriate pillow for the patient. In the nursing process, the use of air bed, every two hours to turn over, each time turning over the nurse to the patient's whole body skin condition check, observe the integrity of the skin. Strengthen posture management, left decubitus, recumbent, right decubitus alternately. 2.1.1 the control group Cotton pillow: the ordinary rectangular pillow with a length of 70 cm and a width of 40 cm is uniformly purchased by Henan Zhengzhou Angel Clothing Co., LTD. The inner core is 60.5% polyester fiber and 39.5% cotton, which meets the requirements of class C of GB 18401-2010 "National Basic Safety Technical Specification for Textile Products". 2.1.2 water pillow set Medical rubber ice bag (Sapling brand), size 45cm×20.5cm, capacity 3L), in line with the national market standard, size 45cm×20.5cm, capacity 3L. After washing, soaking, disinfecting, and drying, put 1500 ~ 2000 mL tap water into the bag. It is appropriate to put the water on the table with a fist on the convex surface of the heat preservation bag without touching the table. Exhaust the air in the bag, tie the water inlet firmly, and put a double layer of cotton pillowcase on the outside. 2.1.3 Gel pillow group It is made of Nissan GELTRON gel pillow, which meets the national standard. The size is 60cm×33cm× 5.5-7.5cm, and the weight is 3kg. The inner material is gel, high permeability sponge and high density sponge. The outer material is 60% polyester fiber, 35% tencel and 5% polyurethane.

纳入标准：

Inclusion criteria

排除标准：

1. 患者有精神性疾病；
2. 患者枕部已发生压疮；
3. 患者骨盆骨折、颈椎骨折等无法翻身；
4. 患者极度消瘦：BMI＜15 kg/m2；
5. 患者正在参与其他课题；
6. 患者躁动不安，RASS镇静评分≥2分；
7. 头部敷料包扎无法观察枕部情况。

Exclusion criteria：

1. The patient has a mental illness;
2. Pressure sores have occurred on the patient's occiput;
3. Patients with pelvic fractures, cervical spine fractures, etc. cannot turn over;
4. The patient is extremely emaciated: BMI < 15 kg/m2;
5. The patient is participating in other subjects;
6. The patient is restless, and the RASS sedation score is >= 2;
7. The condition of the occiput cannot be observed when the head is bandaged.
