研究目的：

本研究旨在通过探索适合社区的脑血管病特色专科与全科团队融合服务模式，对脑卒中患者进行规范管理，以期改善其远期预后，提高生活质量，降低疾病负担。  

Objectives of Study：

The purpose of this study is to standardize the management of stroke patients by exploring the integration service mode of cerebrovascular disease specialty and general practice team suitable for the community, so as to improve the long-term prognosis, improve the quality of life and reduce the disease burden.

药物成份或治疗方案详述：

1. 对照组采用常规模式管理，措施包括： （1）门诊全科医生常规诊治及管理； （2）进行常规健康教育； （3）定期随访； （4）签约团队每3个月对患者进行一次常规评估。 2. 观察组采用脑血管病特色专科与全科融合服务模式管理，措施包括： （1）接受脑血管病特色专科与全科联合门诊的诊治及管理。联合门诊由1全科医师和1名天坛医院神经内科医师共同出诊，共同对患者做出疾病决策； （2）接受脑卒中相关培训的全科医生和护士进行脑卒中相关健康教育； （3）定期随访； （4）专科医生，全科团队（全科医生，护士、社区中医药专家，药师，康复师、检验技师）每3个月对患者进行一次评估。 

Description for medicine or protocol of treatment in detail：

1. The control group was managed in the conventional mode, and the measures included: (1) Routine diagnosis, treatment and management of outpatient general practitioners; (2) Routine health education; (3) Regular follow-up; (4) The contracted team conducted routine visits to patients every 3 months. 2. The observation group was managed by the integrated service model of cerebrovascular disease specialty and general practice. (1) In the joint outpatient clinic, a general practitioner and a neurologist from Tiantan Hospital jointly visit and make disease decisions for patients; (2) General practitioners and nurses who have received stroke-related training provide stroke-related health education; (3) Regular follow-up; (4) Specialists and general practice teams (general practitioners, nurses, community TCM experts, pharmacists, rehabilitation specialists, laboratory technicians) evaluate patients every 3 months. 

纳入标准：

Inclusion criteria

排除标准：

1. 病情危重，无法配合相关检查、测试者；
2. 疾病终末期，预期寿命≤12个月；
3. 存在严重认知功能障碍或完全失能、卧床的患者；
4. 合并有急性或/和严重的肝、肾、肿瘤和造血系统等疾病；
5. 正在参加其他与本研究相近的研究。

Exclusion criteria：

1. Those who are in critical condition and unable to cooperate with relevant examinations and tests;
2. Those who are in the terminal stage of the disease, with a life expectancy of <=12 months;
3. Those who have severe cognitive impairment or total disability and are bedridden;
4. Combined with acute or/or severe liver, kidney, tumor, and hematopoietic system diseases;
5. Are participating in other studies similar to this study.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

小组成员的培训和讨论分析采用录音的方式留存资料；对脑卒中患者的评估数据采取现场一对一采集，将所收集的资料进行整理、编码，采用平行双录入方式录入Excel保存，并进行交叉核对。两类数据由专人负责保存、监督、统计。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

The training, discussion and analysis of the team members are recorded by recording; the assessment data of stroke patients is collected one-on-one on the spot, the collected data are sorted and coded, and the data is entered into Excel by parallel double entry for preservation, and cross-cut. check.
