研究目的：

本研究拟通过在坐骨神经阻滞+股神经阻滞下行全膝关节置换术时，在局麻药物中添加地塞米松，评价其对术后爆发痛发生率及其严重程度的影响效果。

Objectives of Study：

To explore the effect of perineural dexamethasone on rebound pain after ropivacaine single injection combined femoral and sciatic nerve block for total knee arthroplasty.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

Inclusion criteria

排除标准：

1. 对试验所涉及的药物过敏（地塞米松，罗哌卡因，全身麻醉所需药物，阿片类镇痛药物，非甾体类抗炎药等）；
2. BMI≥40；
3. 长期使用镇痛药物；
4. 有外周神经阻滞禁忌症；
5. 长期使用糖皮质激素；
6. 神经阻滞失败（在复苏室中NRS评分＞3）。

Exclusion criteria：

1. Allergies to any of the study drugs (dexamethasone, ropivacaine, general anesthetics, opioids, NSAIDs);
2. BMI>=40;
3. Chronic analgesic use;
4. Contraindication of peripheral nerve block;
5. Chronic steroid use;
6. Failed nerve block(NRS > 3 in the recovery room).
