研究目的:

本研究旨在从症状学、解剖学和脑功能网络三个层面寻找并证实认知障碍与低灌注脑区、脑网络连接、拓扑属性异常的对应关系,不仅为认知障碍提供客观依据,同时建立完善的评价体系: 1.在症状学层面证实慢性脑缺血与患者认知功能障碍具有相关性,并区分不同认知功能损害程度。 2.在脑解剖学层面定位不同认知功能障碍的相关缺血低灌注脑区,为静息态脑网络分析提供ROI(感兴趣脑区)。 3.在脑功能网络层面定位不同认知功能高相关度的默认网络连接及节点异常;通过对入组缺血性疾病患者的外科干预,反证颅内脑血流灌注改善对相关认知障碍及默认网络、拓扑属性的影响;据此初步阐述凭借外科手术干预前后缺血性脑血管病患者脑组织灌注与认知的动态变化,证明静息态脑网络在认知功能损害及重塑中的作用及其机制。 4.在认知功能和脑默认网络、拓扑属性的评价基础上,为颈内动脉系统慢性缺血性疾病提出脑功能层面的评估体系,作为此类疾病的治疗指证及预后评价基础。  

Objectives of Study:

The purpose of this study is to find and confirm the corresponding relationship between cognitive impairment and hypoperfusion brain areas, brain network connectivity and topological properties from the three levels of symptomatology, anatomy and brain functional network, so as to provide an objective basis for cognitive impairment and establish a perfect evaluation system: 1. To confirm the correlation between chronic cerebral ischemia and cognitive impairment at the symptomatic level, and to distinguish different degrees of cognitive impairment. 2. To locate the ischemia-hypoperfusion brain regions related to different cognitive impairment at the brain anatomical level, and to provide ROI (region of interest) for resting-state brain network analysis. 3. Locate the default network connectivity and node abnormalities with high correlation degree of different cognitive functions at the brain functional network level; Through the surgical intervention of patients with ischemic diseases, the effects of improved intracranial cerebral blood perfusion on related cognitive impairment and default mode network and topological properties were proved. Therefore, the dynamic changes of brain tissue perfusion and cognition in patients with ischemic cerebrovascular disease before and after surgical intervention were preliminarily described, and the role and mechanism of resting-state brain network in cognitive impairment and remodeling were proved. 4. Based on the evaluation of cognitive function and brain default mode network and topological properties, a brain function level evaluation system is proposed for chronic ischemic diseases of the internal carotid artery system, as a treatment indication and prognosis evaluation basis for such diseases.

Inclusion criteria

排除标准:

1.免疫系统疾病导致血管病变的患者。
2.不能接受血运重建手术治疗的患者。
3.存在其它影响认知功能的神经精神疾病(如阿尔茨海默症,帕金森症,抑郁症,焦虑症,脑出血,脑积水,颅脑外伤等)。
4.具有磁共振检查的禁忌症(带有心脏起搏器、神经刺激器者、人工金属心脏瓣膜等其他金属异物者等)。

Exclusion criteria:

1. Patients with vascular disease due to immune system disorders.
2. Patients unable to undergo revascularization surgery.
3. Other neuropsychiatric diseases affecting cognitive function (such as Alzheimer's disease, Parkinson's disease, depression, anxiety, cerebral hemorrhage, hydrocephalus, craniocerebral trauma, etc.).
4. Contraindications to magnetic resonance examination (with cardiac pacemaker, nerve stimulator, artificial metal heart valve and other metal foreign body, etc.).
