研究目的:

在牙颌面畸形的诊治过程中,骨性Ⅱ类错牙合畸形患者常常伴发颞下颌关节不稳定现象,为术中颌骨定位带来困难。针对此类型患者,目前临床多采用“下颌优先”的手术模式来提高手术精确性。本研究旨在探索三维打印的个性化钛合金截骨/接骨板对增加骨性Ⅱ类患者正颌手术精确性的影响,并与传统牙合板引导下的“下颌优先”手术模式作对比,通过临床随机对照研究,比较两种手术方式对手术精确性的影响及优劣,解决在此问题上的争议,从而为骨性Ⅱ类错牙合畸形患者提供更具针对性的治疗方案。  

Objectives of Study:

In the diagnosis and treatment of dento-maxillofacial deformities, patients with skeletal class II malocclusion are often accompanied by instability of the temporomandibular joint (TMJ), which brings difficulties to the positioning of the jaw during surgery. The mandible-first surgery is currently used in clinical practice to improve the accuracy of surgery for this type of patient. The purpose of this study is to explore the influence of 3D printed customize-made titanium osteotomy and osteosynthesis plate on increasing the accuracy of orthognathic surgery in patients with skeletal class II malocclusion and to compare it with the mandible-first surgery guided by the traditional occlusal splint. Comparing two different methods on the accuracy of orthognathic surgery by prospective randomized controlled clinical study. Comparing the merits of the two surgical methods to realize the optimization and improvement of orthognathic surgery.

纳入标准:

Inclusion criteria

排除标准:

1. 非首次进行正颌手术,既往有上、下颌骨手术史;
2. 既往或目前存在上下颌骨创伤,肿瘤或软组织缺损;
3. 正颌手术方案是单颌或包含分块截骨手术;
4. 诊断属于颅颌面综合征;
5. 对钛板、钛钉等植入物过敏,难以对内植物的有效性和安全性做出确切评价的患者;
6. 急性或严重感染或慢性感染处于急性发作期的患者;
7. 放射性骨髓炎、急慢性骨髓炎、骨质疏松患者;
8. 妊娠、哺乳期女性;
9. 合并心、肺、脑、肾等重要脏器原发性疾病未控制的患者;
10. 精神疾病患者。

Exclusion criteria:

1. With a history of surgery to the maxilla and mandible;
2. Previous or current with jaw trauma, tumor, or soft tissue defect;
3. Orthognathic surgical plan is single-jaw surgery or includes segmental osteotomy;
4. Diagnosis with craniofacial anomaly syndrome;
5. With reject reaction to the titanium plate, titanium nails, and other implants;
6. With acute severe infection or chronic infection in the acute stage;
7. With radiation-induced osteomyelitis, acute and chronic osteomyelitis, and osteoporosis;
8. Pregnant or lactating females;
9. Patients were complicated with uncontrolled substantial diseases of important organs such as the heart and kidney;
10. With mental illness.
