研究目的：

采用传统会阴柱作为下肢牵引对抗装置进行髋关节镜手术可能发生的牵引相关并发症包括会阴部神经麻痹，功能障碍、会阴区皮肤软组织损伤等。新型的牵引技术采用摩擦海绵固定患者躯干代替会阴柱提供牵引对抗，可能减少牵引相关并发症。探究两种牵引技术对髋关节镜手术操作和并发症发生率的影响，从而为指导临床治疗提供依据。

Objectives of Study：

The possible traction-related complications of hip arthroscopy with a perineal post as lower extremity traction resistance device include perineal nerve palsy, dysfunction, and perineal skin and soft tissue damage. Novel traction techniques use friction sponges to immobilize the patient's torso instead of the perineal post to provide traction resistance, potentially reducing traction-related complications. This study explored the effect of two traction techniques on hip arthroscopy operation and the incidence of complications, so as to provide a basis for guiding clinical treatment.

药物成份或治疗方案详述：

以山东大学第二医院关机外科/运动医学科需要髋关节镜手术治疗的患者为研究对象，采用前瞻性、随机对照研究设计，共计观察160例患者，根据纳入标准将纳入病例随机分入无会阴柱牵引治疗组（实验组，N）及会阴柱牵引治疗组（对照组，Y）。 Y组（会阴柱组）：患者取仰卧位，术前采用牵引架牵引患肢，使用会阴柱，足部牵引套中衬垫充足的棉垫以避免对足部造成损伤，通常以绑带固定加固。 N组（无会阴柱组）：患者取仰卧位，术前采用牵引架牵引患肢，患者身下使用透气海绵垫，双上肢使用绑带固定于胸前。足部牵引套中衬垫充足的棉垫以避免对足部造成损伤，以绑带固定加固。牵引前X线透视定位以明确髋关节间隙所在位置，牵引至髋关节间隙增大，显示出“空气征”。所有髋关节镜手术均由同一高年资医师进行操作，所有患者均在髋关节牵引状态下进行髋关节镜中央室操作，外周室操作则在非牵引状态下屈髋45°完成。所有患者术后口服扶他林以预防髋关节周围异位骨化。

Description for medicine or protocol of treatment in detail：

A total of 160 patients were observed in a prospective, randomized controlled study design, and the included cases were randomly divided into those without perineum according to the inclusion criteria. Column traction treatment group (experimental group, N) and perineal column traction treatment group (control group, Y). Group Y (perineal column group): the patient was placed in the supine position, the affected limb was pulled by a traction frame before the operation, the perineal column was used, and the foot traction sleeve was well-padded with cotton pads to avoid damage to the feet, usually fixed with straps reinforcement. Group N (no perineal column group): the patient was placed in the supine position, the affected limb was pulled by a traction frame before the operation, the breathable sponge pad was used under the patient's body, and the upper limbs were fixed on the chest with straps. The foot traction sleeve is well padded with cotton pads to avoid damage to the foot and is secured with straps for reinforcement. X-ray fluoroscopic positioning before traction to identify the location of the hip joint space, traction until the hip joint space increases, showing the "air sign". All hip arthroscopy operations were performed by the same senior physician. All patients underwent hip arthroscopy in the central chamber with hip joint traction, and peripheral chamber operations with hip flexion of 45° without traction. All patients were postoperatively orally administered Voltaren to prevent heterotopic ossification around the hip joint.

排除标准：

1.术前有下肢神经功能障碍、下肢静脉血栓、下肢骨折等影响牵引和术后并发症判断的合并症，2.既往同侧髋部手术史， 3.强直性脊柱炎髋关节僵硬患者，4.中重度髋关节骨关节炎患者（Tonnis>2）

Exclusion criteria：

1. Complications such as lower extremity nerve dysfunction, lower extremity venous thrombosis, and lower extremity fractures that affect the judgment of traction and postoperative complications before surgery, 2. History of previous ipsilateral hip surgery, 3. Patients with ankylosing spondylitis and hip stiffness, 4. Patients with moderate to severe hip osteoarthritis (Tonnis>2)
