研究目的：

评估苯磺酸瑞马唑仑对术前合并认知障碍或有痴呆高危因素的髋部手术老年患者术后认知功能的影响。  

Objectives of Study：

To assess the effect of ramazolam besylate on postoperative cognitive function in older patients with preoperative cognitive impairment or hip surgery with risk factors for dementia.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

Inclusion criteria

排除标准：

1.有椎管内麻醉禁忌：腰椎畸形、腰椎间盘突出症、腰椎骨折、腰椎骨折内固定术后、中枢神经系统感染、凝血功能异常（PT延长3秒以上、APTT延长10秒以上）、血小板＜100×10^9/L、口服/皮下注射/静脉注射抗凝剂等、不能配合完成椎管内穿刺；
2.有苯二氮卓类药物不良反应史；
3.窦性心动过缓、房室传导阻滞等缓慢型心律失常；
4.合并精神疾病、服用精神类药物；
5.长期酗酒及酒精依赖；
6.严重的慢性阻塞性肺疾病；
7.术前SBP＞180mmHg和（或）DBP＞110mmHg、空腹血糖＞10mmol/L，严重的器官功能不全；
8.术后需送ICU继续治疗；
9.椎管内穿刺不成功或因手术需求改全麻；
10.研究者判断不适合参加本试验的其他情况。

Exclusion criteria：

1. There are contraindications to spinal anesthesia: lumbar deformity, lumbar disc herniation, lumbar fracture, lumbar fracture after internal fixation, central nervous system infection, coagulation dysfunction (PT prolonged for more than 3 seconds, APTT prolonged for more than 10 seconds), platelet < 100 X 10^9/L, oral/subcutaneous injection/intravenous anticoagulant, etc., can not cooperate with the completion of spinal puncture;
2. There is a history of adverse reactions to benzodiazepines;
3. Sinus bradycardia, atrioventricular block and other slow arrhythmias;
4. Concomitant mental illness, taking psychotropic drugs;
5. Long-term alcoholism and alcohol dependence;
6. Severe chronic obstructive pulmonary disease;
7. Preoperative SBP > 180 mmHg and/or DBP > 110 mmHg, fasting blood glucose > 10 mmol/L, severe organ dysfunction;
8. Postoperative ICU need to be sent to continue treatment;
9. Intraspinal puncture is unsuccessful or general anesthesia due to surgical needs;
10. Other conditions that investigators judge are not suitable for this trial.
