研究目的：

本研究拟采用混合研究，结合量性研究和质性研究深入探究脑卒中患者社会性回归的特点、变化过程、作用因素，为提升脑卒中患者社会性回归寻找多学科团队可干预点，通过专家咨询制定并实施基于ICF框架（ International classification of functioning, disability and health,简称ICF框架）的阶段性多学科干预方案，以提升康复护理结局，促进患者回归社会。  

Objectives of Study：

This study intends to adopt a mixed study and combine quantitative and qualitative research to further explore the characteristics, change process and influencing factors of stroke patients' social regression, so as to find intervention points for the improvement of stroke patients' social regression by multidisciplinary teams. Develop and implement a phased multidisciplinary intervention program based on the international classification of functioning, disability and health (ICF framework) through expert consultation to improve the outcome of rehabilitation care, to facilitate the reintegration of patients into society.

纳入标准：

Inclusion criteria

排除标准：

1.由其他脑病引起后遗症，如肿瘤、脑外伤、脑寄生虫病等；
2.未引起神经功能缺损后遗症的中风患者，如短暂性脑缺血发作（TIA）等；
3.言语理解障碍或存在严重的听觉障碍或视觉障碍的患者；
4.病情不稳定，身体虚弱，无法耐受等原因无法配合。

Exclusion criteria：

1. Sequelae caused by other encephalopathy, such as tumor, brain trauma, cerebral parasitic disease, etc.;
2. Stroke patients with no sequelae of neurological impairment, such as transient ischemic attack (TIA);
3. Patients with speech comprehension disorder or severe auditory or visual impairment;
4. The condition is unstable, the body is weak, can not tolerate and other reasons can not cooperate.

根据入组患者NIHSS评分结果，分为轻（0-7分）、中（8-20分）、重（＞20分），采用随机区组分组的方法，按1:1分配原则产生随机序列，区组长度=4。

Randomization Procedure (please state who generates the random number sequence and by what method)：

According to the NIHSS scoring results of the enrolled patients, they are divided as light (0-7 points), medium (8-20 points), and heavy (>20 points). The method of grouping by random area is adopted, and the random sequence is generated according to the principle of 1:1 allocation. Block length=4.
