研究目的：

通过在脑卒中患者出院前准备中应用基于赋能理论的协作式自我管理干预，提高脑卒中患者自我效能和自我管理行为，改善其自理能力和抑郁状态，降低医疗资源的利用，减少脑卒中相关并发症的发生，从而改善患者的生活质量。  

Objectives of Study：

The aim of this study is to examine the effectiveness of incorporating a Collaborative Self-Management Intervention during pre-discharge planning on stroke survivors' health and rehabilitation outcomes.

纳入标准：

Inclusion criteria

排除标准：

①失语患者；②预计住院日短于10天；③合并有其他严重疾病；④自理能力重度依赖；⑤住院期间发生转科；⑥参与其他研究。

Exclusion criteria：

1. suffer from aphasia;
2. has severe or life-limiting co-existing disease;
3. has pre-morbid dependence;
4. Transferred to another unit during hospitalization;
5. is currently involving in any other research program.
