研究目的：

本研究目的是评估多学科团队决策模式对老年失动住院患者提供多重干预的效果。我们假设多学科团队干预策略可以改善老年失动患者活动能力、生活质量和营养状况，减少抑郁、失能、虚弱和跌倒的发生，缩短住院时间，并降低死亡率。

Objectives of Study：

The purpose of this study was to evaluate the effect of a multidisciplinary team decision-making model in providing multiple interventions for elderly patients with Mobility Limitation in hospital. We hypothesized that a multidisciplinary team intervention strategy could improve mobility, quality of life, and nutritional status, reduce depression, disability, frailty, and falls, shorten hospital stays, and reduce mortality.

纳入标准：

Inclusion criteria

排除标准：

1. 运动禁忌症或终末期疾病；
2. 严重的认知和精神障碍（MMSE<10分）；
3. 患者或家属无法配合评估和干预随访；
4. 诊断为中度至重度失能。

Exclusion criteria：

1. Exercise contraindications or end-stage disease;
2. Severe cognitive and psychiatric disorders (MMSE <10 points);
3. Patients or family members are unable to cooperate with assessment and intervention follow-up;
4. Diagnosed with moderate to severe disability.
