研究目的：

老年人都会有不同程度的慢性炎症，这种炎症与多种老年病包括心血管疾病、2型糖尿病、骨质疏松以及阿兹海默病等老年病的发病有关1-3。尽管老年慢性炎症的发病机理不明，我们的前期研究证明，老年的表皮具有下列功能异常：通透屏障功能恢复延缓、角质层含水量降低和皮肤表面酸碱度升高4-7，所有这些改变均能导致皮肤炎症。我们最近的研究结果显示8-10，破环表皮通透屏障功能不仅增加皮肤细胞炎症因子的表达，循环血内的细胞炎症因子量也升高。而且，这些细胞炎症因子的表达不依赖与T细胞和肝细胞10。我们进一步的研究结果显示10，老年小鼠皮肤和血细胞炎症因子的表达增高，改善表皮功能可以降低这些小鼠皮肤和血细胞炎症因子的表达。更为重要的是改善表皮功能可以降低老年人血细胞炎症因子的表达。由于慢性炎症与多种老年病的发病有关，因此，从理论上讲，改善炎症可以减轻和预防这些老年病。在此研究，我们拟观察改善表皮功能是否可以预防或减轻老年病2型糖尿病的发生和发展

Objectives of Study：

There are different degrees of chronic inflammation in the elderly, which is associated with 1-3 of the incidence of geriatric diseases, including cardiovascular diseases, type 2 diabetes, osteoporosis and Alzheimer's disease. Although the pathogenesis of chronic inflammation in the elderly is unknown, our previous studies have proved that the epidermis in the elderly has the following functional abnormalities: delayed recovery of permeability barrier function, decreased water content in the stratum corneum and increased pH on the skin surface. 4-7 all these changes can lead to skin inflammation. Our recent results show that 8-10, the function of breaking the epidermal permeability barrier not only increases the expression of inflammatory cytokines in skin cells, but also increases the amount of inflammatory cytokines in circulating blood. Moreover, the expression of these cytokines is independent of T cells and hepatocytes 10. Our further results showed that 10. The expression of inflammatory factors in skin and blood cells of elderly mice increased, and improving epidermal function could reduce the expression of inflammatory factors in skin and blood cells of these mice. More importantly, improving epidermal function can reduce the expression of blood inflammatory factors in the elderly. Because chronic inflammation is related to the pathogenesis of a variety of geriatric diseases, theoretically, improving inflammation can reduce and prevent these geriatric diseases. In this study, we intend to observe whether improving epidermal function can prevent or reduce the occurrence and development of type 2 diabetes in geriatric patients.
