探索在互联网+糖尿病共同照护管理模式下的线上管理中，基于BCT的优化线上管理策略是否比常规线上管理更有效地改善患者自我管理能力。  

Objectives of Study：

To explore whether the optimized online management strategy based on BCT is more effective than conventional online management in improving patients' self-management ability in the online management mode of Internet + diabetes co-care management.

Inclusion criteria

排除标准：

1、不能配合并完成研究的患者；
2、经医师判断具有心理及精神疾患的患者。

Exclusion criteria：

1. Patients who cannot cooperate and complete the study;
2. Patients with psychological and mental disorders judged by doctors.
