研究目的：

在社区中组织建立不同角色组长的慢性病自我管理小组，通过比较社工组、医务人员组以及对照组，评估患者的健康行为以及相关指标，为实现多部门协助下的慢性病患者自我管理，降低患者经济和心理负担，减轻疾病负担提供科学依据。  

Objectives of Study：

To compare social worker leader, health care professional leader outreach intervention and controls in sustaining improvements in non-communicable disease related measurements after self-management models, to identify which approach is appropriate to be promoted across the city currently.

纳入标准：

Inclusion criteria

排除标准：

1.有物质滥用或依赖史者； 2.妊娠期妇女； 3.有严重躯体疾病； 4. 精神疾病； 5.脑部疾病；6. 癌症；7. 语言障碍；8. 有慢性病严重并发症；9. 在过去一年中曾参加另一项干预性临床研究者。

Exclusion criteria：

1. History of drug abuse or dependence;
2. pregnant;
3. serious physical illness;
4. mental illness;
5. brain disorders;
6. cancer;
7. language barriers;
8. serious complications of chronic diseases;
9. had participated in another self-management intervention.
