研究目的：

本研究目的是评估多学科团队决策模式对老年失智住院患者提供多重干预的效果。我们假设多学科团队干预策略，可能改善老年患者失智状况和生活质量，抑郁和功能状况、减少衰弱发生、缩短住院时间。  

Objectives of Study：

This study aims to assess the efficacy of a mullidisciplinary team decision-making model for multiple interventions for aged, mainourished inpatients. We hypothesize that the mullidisciplinary team intervention strategies may improve dementia status,depression, functional status, and quality of life; reduce the incidence of frailty; ande shorten hospital stays.

纳入标准：

Inclusion criteria

排除标准：

1.精神、意识障碍，无法进行有效沟通交流；
2.患者依从性差，无法配合完成评估与干预；
3.患者本人及家属拒绝参与本研究；
4.疾病终末期；
5.急性心肌梗死；
6.剥离性主动脉瘤；
7.重度主动脉瓣狭窄；
8.心内膜炎/急性心包膜炎；
9.急性血栓栓塞症；
10.近期大手术或过去1个月发生肢体骨折者；
11.存在研究者认为需排除的其他状况。

Exclusion criteria：

1. Mental and consciousness disorders, unable to communicate effectively;
2. The patient's compliance is poor, unable to cooperate in the evaluation and intervention;
3. The patient and his family refused to participate in the study;
4. End stage of disease;
5. Acute myocardial infarction;
6. Dissected aortic aneurysm;
7. Severe aortic stenosis;
8. Endocarditis/acute pericarditis;
9. Acute thromboembolism;
10. Recent major surgery or limb fracture in the past month;
11. There are other conditions that the researcher thinks should be excluded.
