研究目的：

通过研究睡眠时相与血糖波动之间规律，分析糖尿病患者的睡眠时相与血糖控制是否存在相关性。如二者之间存在相关性，制定社区具体睡眠干预方案，评价干预方案对改善睡眠及减少血糖波动的效果。  

Objectives of Study：

By studying the pattern between sleep phase and blood glucose fluctuation, we analyzed whether there is a correlation between sleep phase and blood glucose control in diabetic patients. If there is a correlation between them, a community specific sleep intervention program will be developed and the effect of the intervention program on improving sleep and reducing blood glucose fluctuations will be evaluated.

纳入标准：

Inclusion criteria

排除标准：

1. 继发性糖尿病、妊娠期糖尿病、特殊类型糖尿病；
2. 目前存在糖尿病急性并发症，如糖尿病酮症、高糖高渗状态、乳酸酸中毒等；
3. 入组前6月内发生过严重心脑血管事件，包括急性心肌梗死、不稳定型心绞痛、控制不良的心律失常、严重心力衰竭及新发的脑血管意外或既往脑血管疾病遗留严重后遗症者；
4. 药物控制不佳的严重高血压（收缩压>160mmHg或(和)舒张压>100mmHg）；
5. 合并系统性红斑狼疮、皮肌炎、类风湿关节炎等自身免疫疾病者；
6. 长期服用糖皮质激素药物或目前存在妊娠、哺乳、外伤、急性感染等影响空腹血糖或胰岛素敏感性的因素；
7. 慢性肾脏或肝脏疾病者，包括ALT、AST升高超过参考值上限3倍及以上、eGFR<60ml/min/1.73m2；
8. 不规律服用镇静催眠药物者；
9. 合并其他可能影响血糖的疾病或应用可能影响血糖的药物，如垂体疾病、甲状腺疾病、肾上腺疾病、多囊卵巢综合征、贫血（血红蛋白男性<120g/l，女性<110g/l）、失血或（和）输血、应用大剂量维生素C或红细胞生成素等；
10. 合并精神类疾病者或试验方案可能增加其心理不适者，明显认知行为障碍；
11. 严重的阻塞性睡眠呼吸暂停综合征。

Exclusion criteria：

1. Secondary diabetes mellitus, gestational diabetes mellitus, and special types of diabetes;
2. Current acute complications of diabetes, such as diabetic ketosis, hyperglycemic hyperosmolar state, lactic acidosis, etc;
3. Serious cardiovascular and cerebrovascular events within 6 months prior to enrollment, including acute myocardial infarction, unstable angina, poorly controlled arrhythmias, severe heart failure and new cerebrovascular accidents or serious sequelae left from previous cerebrovascular disease;
4. Severe hypertension (systolic blood pressure > 160 mmHg or (and) diastolic blood pressure >100 mmHg) that is poorly controlled by medication;
5. Patients with systemic lupus erythematosus, dermatomyositis, rheumatoid arthritis and other autoimmune diseases;
6. Long-term use of glucocorticoid medication or the current presence of pregnancy, lactation, trauma, acute infection, and other factors affecting fasting glucose or insulin sensitivity;
7. Those with chronic kidney or liver disease, including those with ALT and AST elevated more than 3 times the upper limit of the reference value or more, and eGFR <60ml/min/1.73m2;
8. Those who take sedative-hypnotic drugs irregularly;
9. Combination of other diseases that may affect blood glucose or application of drugs that may affect blood glucose, such as pituitary disease, thyroid disease, adrenal disease, polycystic ovary syndrome, anemia (hemoglobin <120g/l for men and <110g/l for women), blood loss or (and) blood transfusion, application of high-dose vitamin C or erythropoietin, etc;
10. Those with comorbid psychiatric disorders or those whose test protocol may increase their psychological discomfort, and significant cognitive-behavioral impairment;
11. Severe obstructive sleep apnea syndrome.
