研究目的：

本研究筛查门诊/住院的糖尿病足高危患者，对其进行高危因素的干预并随访，以期减少糖尿病足溃疡的发生，并根据随访结果建立门诊糖尿病足预防的具体干预及随访方案。  

Objectives of Study：

In order to reduce the incidence of diabetic foot ulcers, we screen high risk of diabetes, then give target persons follow up and intervention. Finally, to establish a outpatient diabetic foot prevention strategy.

纳入标准：

Inclusion criteria

排除标准：

1）目前正有足溃疡及其他重症疾病；
2）不能自主行走的病人；

Exclusion criteria：

1. Now severe foot ulcers and other systematic diseases;
2. Patient can not walk on their own foot.
