研究目的:

明确CSVD脑血流量对白质微结构损伤的时序关系,揭示灌注在发病机制中的关键作用,建立特定脑区与认知域之间的关联模式,从而阐明“特定脑区-灌注不足-白质损伤-认知障碍”序贯病理机制,为CSVD-CI的有效治疗提供新的理论依据和治疗靶点。  

Objectives of Study:

We want to explicit the sequential change relationship between cerebral blood flow and white matter microstructure in CSVD, which will reveal the key role of perfusion in pathogenesis in CSVD, and find specific brain regions associated with cognitive domains. This study will explain the content of the "specific brain regions-hypoperfusion-white matter lesions-cognitive impairment " pathological mechanism, which will provide theoretical basis and new therapeutic targets for CSVD-CI.

药物成份或治疗方案详述:

本项目为观察性研究设计,不对干预进行特殊限定,所有CSVD患者诊疗方案均参考《中国脑小血管病诊治专家共识2021》中的推荐意见。 

Description for medicine or protocol of treatment in detail:

This study is an observational study design without special restrictions on intervention.The diagnosis and treatment of CSVD are based on the recommendations in "Chinese Expert Consensus on Diagnosis and Treatment of Small Cerebral Vascular Diseases 2021".  

纳入标准:

Inclusion criteria

排除标准:

1. 认知障碍与白质高信号无明显关联如由急性卒中阿尔茨海默病帕金森病等导致;
2. 明确脑卒中病史,包括脑梗塞、脑出血、蛛网膜下腔出血;
3. 经MRA、CTA、DSA等影像学评估颅内外血管狭窄程度≥50%;
4. 非血管因素导致的白质高信号,如结节病、多发性硬化、中毒、脑白质营养不良、脑放疗损伤等;
5. 单基因脑小血管相关遗传类疾病,如伴皮质下梗死和白质脑病的常染色体显性遗传性脑动脉病、淀粉样脑血管病等;
6. MRI提示颞叶萎缩,内侧颞叶萎缩评定量表 (Medial Temporal lobe Atrophy,MTA)评分>2分;
7. 合并阿尔茨海默、额颞叶痴呆、路易体痴呆、帕金森病等神经变性病;
8. 合并脑肿瘤、癫痫、脑外伤等其他神经系统疾病或精神类疾病;
9. 伴随明显运动、感觉、语言、情绪、视听等其他神经系统功能异常;
10. 不能配合完成相关检查或者神经功能评估的患者;
11. 接受心脏起搏器手术、冠状动脉支架置入术、冠状动脉外科搭桥手术、或体内有其他金属物品者;
12. 无法配合完成30分钟以上的MRI检查;
13. 患有严重的器质性疾病,如恶性肿瘤,预期生存时间<2年。

Exclusion criteria:

1. Cognitive impairment is not significantly associated with white matter hyperintensity, such as stroke, Alzheimer's disease, Parkinson's disease, etc.;
2. Previous history of stroke, including cerebral infarction, cerebral hemorrhage, and subarachnoid hemorrhage;
3. The intracranial and extracranial vascular stenosis ≥50%, which is evaluated by MRA, CTA, DSA, and other imaging techniques;
4. White matter hyperintensity are caused by non-vascular factors, such as sarcoidosis, multiple sclerosis, toxication, leukodystrophy, radiation encephalopathy, etc.
5. Single-gene genetic diseases related to small vessels disease, such as CADASIL, CAA, etc.;
6. MRI show the temporal lobe atrophy, and the scores of Medial Temporal lobe Atrophy (MTA)> 2;
7. Comorbidity with alzheimer's disease, frontotemporal dementia, Lewy body dementia, Parkinson's disease, and other neurodegenerative diseases;
8. Comorbidity with brain tumor, epilepsy, brain trauma, and other neurological diseases or mental diseases;
9. Comorbidity with obvious abnormalities in motor, sensory, language, emotion, audio-visual and other neurological;
10. Patients cannot cooperate with relevant examinations or neurological function assessment;
11. Patients have received cardiac pacemaker surgery, coronary artery stand interpolation, coronary artery bypass surgery, or other metal objects in their bodies;
12. Unable to complete MRI examination for more than 30 minutes;
13. Comorbidity with severe disease, such as malignant tumor, the expected survival time is less than 2 years.
