研究目的:

探究神经肌肉训练对ACL重建术后患者关节活动度、本体感觉、平衡功能及脑功能的影响  

Objectives of Study:

To explore the effects of neuromuscular training on range of motion, proprioception, balance and brain function after ACL reconstruction

药物成份或治疗方案详述:

1、神经肌肉训练 根据患者的不同膝关节功能状态,进行神经肌肉训练,具体内容包括: (1)肌肉力量训练:术后4至6周进行闭链肌力训练,8周后进行腘绳肌和股四头肌抗阻训练,术后3至6个月可进行等速肌力训练,术后负重训练应在第6周负重25%体重,第7周负50%体重,第8周负75%体重,手术3个月后可以完全负重,主要练习方法有半蹲训练、下肢功率自行车、等速肌力训练,单腿站立训练等。 (2)平衡功能及动态关节稳定性训练:主要有平衡板训练、单腿站立训练、弓箭步练习。根据患者具体情况,可先选择静态平衡训练,可以按“双足一健足一患足一双足”进行。可增加动态平衡训练,如抛球练习、各方向触及练习、平衡垫练习、摆动板练习等。 (3)灵敏性训练:曲线跨越、90度旋转步、正向步态训练、反向步态训练、侧向步态训练、斜向步态训练、跳跃训练等。 (4)近端控制训练:膝关节近端控制训练是指在运动过程中通过力量和适当的神经控制改善膝关节近端组织结构稳定性的一种训练方法,包括髋关节控制训练、上肢力量训练以及核心稳定性训练如平板支撑、斜向平板支撑、卷腹等。 2、常规康复训练 常规康复训练主要为肌力训练及关节活动度训练,各阶段关节活动度及肌肉力量训练,严格遵照ACL运动康复循证共识中前交叉韧带重建术后康复方案进行。肌力训练主要训练股四头肌、腘绳肌、臀中肌和腓肠肌。关节活动度训练包括伸膝屈膝训练、直推抬高训练以及下肢功率自行车。常规康复训练还包括其他训练方法例如:单腿站立训练、下蹲训练、弓箭步训练、步行训练等。 

Description for medicine or protocol of treatment in detail:

Neuromuscular training According to patients condition, neuromuscular training will be applied to patients. (1) Muscle strength training: close-chain training will start at 4-6 weeks post-surgery; resistance training will start at 8 weeks post-surgery; 3-6 months after operation, isokinetic strength training will be applied. Weight-bearing exercises will start at the 6th week post-surgery, and patients may not need to use orthosis at the 3rd month post-surgery. Muscle strength training includes squatting exercises, bicycle training, isokinetic strength trainingsingle leg stance etc. (2)Balance and dynamic joint stability training: wobble board exercises, single leg standing, lunge exercise etc. Static balance training may be used at the beginning. Step-up with dynamic joint stability training, such as ball throwing, leg or arm reaching, balance mat exercise, wobble board exercise etc. (3)Agility Training: crossing trainingwalking exercises, backwards walking, figure-of-eight running, stop-turn-run running, lateral step over exercise. Jumping exercise includes up and down jumping, vertical jumping etc. (4)Proximal control exercise: focus on enhancing stability of segments proximal to the knee joint through strengthening and learning adequate control in dynamic movements. Proximal control exercise includes hip control exercises, upper body weight training and core stability training such as the plank, side plank, abdominal curl etc. Conventional rehabilitation training Conventional rehabilitation training includes muscle strength training and range of motion exercises. All exercises are based on American College of Sports Medicine (ACSM) recommendations. The muscle strength training is focus on the quadriceps femoris, hamstring, gluteus medius, and gastrocnemius muscles. range of motion exercises include knee flexionknee extensionstraight leg raisingbiking. Conventional rehabilitation training also includes other training programs such as single leg standing exercises, squatting exercises, lunge exercise and walking exercises. 

纳入标准:

Inclusion criteria

排除标准:

1.合并有膝关节其他结构严重损伤,例如后交叉韧带损伤、髌骨骨折等;
2.MRI显示ACL附着点处有撕裂性骨折;
3.严重视力缺陷或者前庭功能障碍患者;
4.合并心、脑、肾及造血系统严重疾患者;
5.肿瘤、妊娠及哺乳者;
6.既往有脑血管意外患者;
7.试验开始前6个月内加入了其他临床试验者;
8.研究者认为可能存在增加患者危险性或干扰临床试验的任何情况,不适合进入本研究者。

Exclusion criteria:

1. Complicated with serious injuries to other structures of the knee joint, such as posterior cruciate ligament injury, patella fracture, etc;
2. MRI showed tear fracture at ACL attachment point;
3. Patients with severe visual impairment or vestibular dysfunction;
4. Patients with serious diseases of heart, brain, kidney and hematopoietic system;
5. Tumor, pregnancy and lactation;
6. Patients with cerebrovascular accident in the past;
7. Other clinical experimenters were added within 6 months before the start of the trial;
8. The investigator believes that there may be any situation that may increase the risk of patients or interfere with the clinical trial, which is not suitable for the investigator.
