研究目的：

通过多学科团队合作构建脑卒中后吞咽障碍患者个体化营养管理方案，早期识别不同程度吞咽障碍的患者，给予个体化的营养管理及连续性护理，改善患者的营养状况，降低吸入性肺炎等并发症的发生率，促进神经功能的恢复，减少患者经济负担。  

Objectives of Study：

Through multi-disciplinary team cooperation, build an individualized nutrition management scheme for patients with post-stroke dysphagia, early identify patients with different degrees of dysphagia, give individualized nutrition management and continuous nursing, improve the nutritional status of patients, reduce the incidence of complications such as aspiration pneumonia, promote the recovery of neurological function and reduce the economic burden of patients.

纳入标准：

Inclusion criteria

排除标准：

1.既往存在吞咽异常的患者；
2.胃肠功能明显不正常，频繁呕吐或腹泻的患者或存在肠内营养禁忌症的患者；
3.入院前已行肠内营养支持的患者；
4.依从性差,不能完成随访，临床资料不全及其他研究者认为不适宜入组的情况；
5.患者及直系亲属不愿入组。

Exclusion criteria：

1. Patients with previous abnormal swallowing;
2. Patients with significantly abnormal gastrointestinal function, frequent vomiting or diarrhea, or patients with contraindications to enteral nutrition;
3. Patients who have received enteral nutrition support before admission;
4. Poor compliance, unable to complete the follow-up, incomplete clinical data, and other researchers who think it is not suitable for enrollment;
5. Patients and immediate family members are unwilling to join the group.
