研究目的：

1.通过对近5年急诊多发伤患者的回顾性分析，试图使用入院第一时间客观的临床实验室指标，来开发并构建一种急诊多发伤DVT形成的风险模型； 2.通过内部验证及外部比较来评估创建的新模型的临床预测能力； 3.根据DRAM模型对急诊多发伤患者进行DVT风险分层，前瞻性评估急诊多发伤患者DVT发生的风险，进而用于指导临床抗凝治疗的药物剂量与药物时间的选择。

Objectives of Study：

1. Through the retrospective analysis of the emergency multiple trauma patients in the past 5 years, try to use the objective clinical laboratory indicators at the first time of admission to develop and construct a risk model for the formation of emergency multiple trauma DVT; 2. Evaluate the clinical predictive ability of the new model created through internal validation and external comparison; 3. According to the DRAM model, DVT risk stratification was performed on emergency patients with multiple trauma, and the risk of DVT in emergency multiple trauma patients was prospectively assessed, and then used to guide the choice of drug dose and drug time for clinical anticoagulation therapy.

纳入标准：

Inclusion criteria

排除标准：

1.无下肢血管彩超；
2.既往有严重肝肾功能不全、恶性肿瘤、凝血功能异常病史；
3.入院前1周曾口服抗凝、抗血小板药物及口服避孕药的患者。

Exclusion criteria：

1. No color Doppler ultrasound of lower extremity blood vessels;
2. History of severe liver and kidney insufficiency, malignant tumor, abnormal coagulation function in the past;
3. Patients who had taken oral anticoagulation, antiplatelet drugs and oral contraceptives one week before admission.

随机方法（请说明由何人用什么方法产生随机序列）：

将入组患者分为两组：有DVT组及无DVT组患者；有DVT组再分为两个亚组：近端DVT组及非近端DVT组；对以上搜集临床资料进行单因素分析，比较两组及亚组间基线特征及预后情况。

Randomization Procedure (please state who generates the random number sequence and by what method)：

采用LASSO技术从所有可能的危险因素中筛选出具有最佳预测价值的回顾性创伤队列变量。经LASSO回归后，假设有8个危险变量纳入nomogram预测模型，构建急诊多发伤DVT的DRAM模型。依据DRAM风险评分，假设有8个数据纳入模型风险评分。将模型危险分层：低危组（DRAM评分1-2分）、中危组（DRAM评分3-4分）、高危组（DRAM评分5-8分）。预搜集2022年2月至7月的急诊多发伤病人（预计200人），选取入院48小时内最差值的8个危险因素指标，并依据模型危险评分分组（低危、中危、高危组），预测患者下肢静脉血栓风险；搜集患者住院3天内以及术后第二天下肢静脉B超结果，进而验证DRAM模型所预测的DVT风险的敏感性及特异性。
