研究目的：

本研究以Triangle慢性疾病分层管理模型为指导，现阶段国家推行的分级诊疗为导向，以提高腹膜透析患者透析质量为目的，通过查阅文献、腹膜透析团队成员反复讨论和多中心的专家咨询，对腹膜透析患者分层分级进行更为科学的评定，并赋予不同的权重系数，构建满足临床所需的分层分级管理并应用于临床腹膜透析管理，优化护理人力资源配置，提高管理效率，达到良好的腹膜透析管理目标。  

Objectives of Study：

This research guided by the hierarchical Triangle chronic disease management model, the present stage national classification of diagnosis and treatment as the guidance, to improve the quality of peritoneal dialysis patients for the purpose, through the consult literature and pd team members discussed repeatedly and multicenter expert consultation, hierarchical classification in patients with peritoneal dialysis in more scientific evaluation, and given different weight coefficient, To construct hierarchical management to meet clinical needs and apply it to clinical peritoneal dialysis management, optimize the allocation of nursing human resources, improve management efficiency, and achieve good peritoneal dialysis management goals.
