研究目的：

早期肝癌的的主要根治性手段有外科手术和肝移植。对于无法行外科根治的患者，应当考虑局部治疗，局部治疗手段主要分为消融、肝动脉介入、外照射放疗。立体定向放射治疗（SBRT）是一种精确放疗手段，可将射线辐射到肿瘤组织，而降低正常组织剂量，以达到消灭肿瘤的目的。NCCN指南推荐SBRT可以替代消融/栓塞技术、或者用于消融/栓塞治疗失败或存在禁忌症的病例。因此我们拟行II期临床研究以评价立体定向放射治疗局部不可手术切除HCC的有效性和安全性。

Objectives of Study：

Surgical resection and liver transplantation are the first-line treatment for early stage HCC patients. Local ablation and TACE are also considered the standard of care for patients at early stages not suitable for surgical therapies. The National Comprehensive Cancer Network (NCCN) introduced SBRT as an alternative for inoperable tumors and the ablation or embolization techniques mentioned above or when these therapies have failed or are contraindicated.Therefore, we are going to phase II clinical study to evaluate the efficacy and safety of SBRT for HCC who are unsuitable for surgical resection or local ablative therapy

纳入标准：

Inclusion criteria

排除标准：

1、既往上腹部放射治疗病史
2、CTP C级；CTP A/B级且病灶数超过3个或直径和超过10cm
3、门脉癌栓或下腔静脉癌栓
4、肿瘤直接侵及周围组织，或致胆囊、其他脏器穿孔
5、区域淋巴结转移或者肝外转移
6、未照射肝体积＜700ml
7、存在放射治疗禁忌
8、食管静脉重度曲张并出血病史或入组2周内出现活动性胃肠道出血
9、明显腹水且临床治疗无效
10、孕妇
11、既往6月内曾行全身化疗
同时进行其他临床试验

Exclusion criteria：

1. Previous radiation therapy of upper abdominal;
2. CTP class C;CTP class A and B, the number of lesions more than 3 or sum diameter is more than 10cm;
3. Portal vein tumor thrombus or inferior vena cava tumor thrombus;
4. Tumor direct invasion and surrounding tissue, or cause gallbladder, other organs perforation;
5. Regional lymph node metastasis or extrahepatic metastasis;
6. No irradiation liver volume < 700ml;
7. There are radiation therapy taboo;
8. Severe esophageal varices and bleeding history or active gastrointestinal hemorrhage befor 2 weeks; 9. Obvious ascites and clinical treatment is invalid;
10. Pregnant women;
11. Systemic chemotherapy in the past six months;
12. Join other clinical trials at the same time.
