研究目的：

1.探讨脑梗死后认知功能障碍的患病率及发病率； 2.寻找脑梗死后认知功能障碍的危险因素； 3.寻找脑梗死后认知功能障碍的早期诊断标志物，包括体液、影像、临床症状、神经心理等标志物； 4.观察缺血性卒中后6月内，认知功能的发展轨迹； 5.为研究卒中与认知障碍搭建规范的平台。  

Objectives of Study：

1. To explore the prevalence and incidence of cognitive dysfunction after cerebral infarction; 2. Look for the risk factors of cognitive dysfunction after cerebral infarction; 3. Look for early diagnostic markers of cognitive dysfunction after cerebral infarction, including markers of body fluids, imaging, clinical symptoms, neuropsychology, etc.; 4. To observe the developmental trajectory of cognitive function within 6 months after ischemic stroke; 5. Build a normative platform for the study of stroke and cognitive impairment.

纳入标准：

Inclusion criteria

排除标准：

1.既往痴呆病史。
2.本次NIHSS评分大于等于15分。
3.存在可引起脑功能障碍的其他神经系统疾病（如抑郁、脑肿瘤、帕金森病、代谢性脑病、脑炎、多发性硬化、癫痫、脑外伤、正常颅压脑积水等）。
4.存在生命体征不稳定或可引起认知障碍的其他系统性疾病：肝、肾功能不全、风湿性关节炎（血清炎症指标异常）、甲状腺功能异常、严重贫血、叶酸和维生素B12缺乏、特殊感染（如梅毒、HIV）、酒精及药物滥用，其他（由团队一起讨论决定）。
5.存在精神和神经发育迟滞。
6.存在其他已知可能导致认知障碍的疾病。
7.严重心肺等脏器疾病（心肌梗死、心衰、肥厚性心肌病）。
8.存在核磁共振禁忌症。
9.罹患无法配合完成认知检查的疾病（失语、利手瘫痪（肌力小于3级））。
10.拒绝抽血。
11.基线期拒绝签署知情同意书。

Exclusion criteria：

1. Past history of dementia.
2. This NIHSS score is greater than or equal to 15 points.
3. There are other neurological diseases that can cause brain dysfunction (such as depression, brain tumor, Parkinson's disease, metabolic encephalopathy, encephalitis, multiple sclerosis, epilepsy, brain trauma, normal intracranial pressure hydrocephalus, etc.).
4. Other systemic diseases with unstable vital signs or cognitive impairment: liver and renal insufficiency, rheumatoid arthritis (abnormal serum inflammatory markers), abnormal thyroid function, severe anemia, folic acid and vitamin B12 deficiency, specific infections (eg, syphilis, HIV), alcohol and drug abuse, others (to be discussed with the team).
5. There is mental and neurodevelopmental delay.
6. The presence of other diseases known to cause cognitive impairment.
7. Severe heart and lung diseases (myocardial infarction, heart failure, hypertrophic cardiomyopathy).
8. There are contraindications to MRI.
9. Suffering from diseases that cannot cooperate with the completion of cognitive examinations (aphasia, hand paralysis (muscle strength less than grade 3).
10. Refusing to draw blood.
11. Refused to sign the informed consent during the baseline period.
