探索构建基于信息化管理平台的失智老人居家照护模式；评价基于信息化管理平台的居家照护模式的实施对出院后失智老人的照护效果。  

Objectives of Study：

Explore the construction of a home care model for the demented elderly based on an information management platform; evaluate the effect of the implementation of the home care model based on the information management platform on the care of the demented elderly after discharge from the hospital.

纳入标准：

Inclusion criteria

排除标准：

1.意识障碍者；
2.严重自杀倾向者；
3.严重耳聋、失语无法交流者；
4.严重失能无法参与活动者。

Exclusion criteria：

1. People with consciousness disorder;
2. Those with serious suicidal tendencies;
3. Those who are seriously deaf or aphasic and unable to communicate;
4. Those who are seriously disabled and unable to participate in activities.
