研究目的:

1、招募脑卒中患者进行临床随机对照试验,验证基于“左右协同与制衡”理论的双侧TEAS联合阶梯式TOT训练对脑卒中患者上肢手功能恢复的影响; 2、建立一套联合双侧TEAS和阶梯式TOT训练且适宜在基层康复机构使用和推广的脑卒中上肢手功能康复方案,该方案需要具备经济易用性和临床有效性的特征,并得到循证证据的支持; 3、通过TMS神经电生理评估技术,分析脑卒中患者在接受干预前后皮质脊髓束兴奋性的变化,从而剖析“双侧TEAS+阶梯式TOT”对脑卒中患者神经功能重塑的影响,并为深入探索脑卒中患者肢体功能恢复的神经机制提供依据。  

Objectives of Study:

1. Recruit stroke patients for clinical randomized controlled trials to verify the effect of bilateral TEAS combined with stepped TOT training based on the theory of ''left and right synergy and balance'' on the recovery of upper limb hand function in stroke patients; 2. Establish a set of stroke upper limb hand function rehabilitation program that combines bilateral TEAS and stepped TOT training and is suitable for use and promotion in grass-roots rehabilitation institutions. The protocol needs to be characterized by economic ease of use and clinical effectiveness, supported by evidence-based evidence; 3. Through the TMS neuroelectrophysiological assessment technology, the changes of corticospinal tract excitability in stroke patients before and after intervention were analyzed, in order to analyze the effect of ''bilateral TEAS + stepped TOT''on the neurological remodeling of stroke patients, and provide a basis for further exploration of the neural mechanism of limb function recovery in stroke patients.

纳入标准:

Inclusion criteria

排除标准:

1.严重的痉挛(改良Ashworth痉挛量表>2分);
2.偏瘫侧肢体明显的疼痛(VAS评分>4分);
3.明显的感觉功能障碍、失语症、偏侧忽略、失用症或视听觉障碍;
4.并发癫痫或严重心、肝、肾和造血系统等重要脏器功能衰竭;
5.存在TMS的禁忌症,包括头颅内置有金属异物、带心脏起搏器者、心脏支架者、有耳蜗植入物者,有颅内压明显增高者等;
6.存在酗酒或药物滥用史;
7.不能完成基本疗程,依从性可能不好者;
8.非本地病患无法随访者。

Exclusion criteria:

1. Severe spasticity (modified Ashworth spasticity scale > 2 points);
2. Obvious pain in the hemiplegic limb (VAS score > 4);
3. Obvious sensory dysfunction, aphasia, lateral neglect, apraxia or visual and hearing impairment;
4. Concurrent epilepsy or severe heart, liver, kidney and hematopoietic system and other important organ failure;
5. There are contraindications to TMS, including those with metal foreign bodies built into the skull, those with pacemakers, those with cardiac stents, those with cochlear implants, those with significantly increased intracranial pressure, etc.;
6. There is a history of alcohol or drug abuse;
7. Those who cannot complete the basic course of treatment and may have poor compliance;
8. Non-local patients who cannot be followed up.
